Job Description Under the supervision of the Administrative Nurse Manager, guided by the policies and procedures of the Ambulatory Care Center, and in collaboration with the physician and other clinical staff, the Clinical Nurse PD is responsible for providing safe, efficient nursing care to patients in an out-patient, primary care practice. Triages patients and responds appropriately according to protocol and nursing assessment. The Clinical Nurse PD utilizes the nursing process (Assessment, Planning, Implementation and Evaluation) to provide safe and therapeutic clinical care. The Clinical Nurse PD also provides clinical support to providers and participates as part of the health care team to assure coordination and continuity of patient care. S/he participates in the development and implementation of educational programs and materials for patients and their family members. S/he delivers direct patient care to a variety of patient populations. All components of nursing care will be individualized to meet the patient’s physiological and psychosocial development. The Clinical Nurse PD will play a critical role in the patient experience including optimizing clinic flow, providing in and outreach to patients and working collaboratively with the team to create a patient centered environment. S/he is responsible for daily clinical operations and maintaining a safe environment for patients and staff. The Clinical Nurse PD consistently projects a professional and courteous demeanor, helps establish and maintain the highest service standards for the practice and complies with the Medical Center House and Telephone Standards. Responsibilities ASSESSMENT Identifies the needs of patients through observations of disease symptoms, emotional state, cultural and socioeconomic background and response to medical regimen and treatment. Collects and organizes data from the client, encouraging them to present their problems or symptoms through active listening and open-ended questions. Assesses each client’s problem(s) based on auditory, verbal, and emotional cues, as well as information extracted from the medical record. Triages and responds or refers inquiries appropriately. Recognizes and reports specific physical and emotional conditions/needs/concerns of the patients to the appropriate members of the health care team. Alerts physician of unusual observations of patients to facilitate immediate attention, e.g. chest pain, shortness of breath. IMPLEMENTATION / EVALUATION Provides individualized nursing care which responds to the special needs of the patient population and demonstrates and understanding of the treatment needs required for patients. Performs nursing intake process according to the standards in the nursing policies and procedure manual. Performs nursing activities as ordered by the physician, including venipunctures and electrocardiograms. Administers medications and thoroughly documents in the medical record. Maintains current knowledge of pharmacology and assure medications are current. Instructs patients on specific diagnostic procedures (specimen collection, etc. ) Assists as needed with diagnostic procedures and treatments. Maintains knowledge about the operation and care of biomedical equipment in the practice . Responds to the emotional needs of patients by demonstrating a sensitivity to their emotional state by relating to patients in an appropriate manner. Refers to appropriate members of the health team as indicated. Responds appropriately to emergencies and maintains knowledge of emergency procedures and supplies available. Complies with all ACC and Medical Center policies and procedures for infection control, safety, administrative, and clinical practice. Complies with activities mandated by The Joint Commission /Title 22. COORDINATION / CONTINUITY / COMMUNICATION Organizes nursing care based on the needs of patients, while maintaining the effective flow of patients through the clinic. Adjusts the clinical support activities to reflect changes in priorities during practice sessions. Maintains open communication and facilitates continuity of care for patients and/or family with members of the health care team through phone contact, verbal interaction and/or written reports. Utilizes electronic healthcare information (i.e. labs, appointments) to contribute to patient care. Assists with coordinating diagnostic testing. Maintains accurate follow-up on results and contacts patient with any significant result requiring treatment ordered by the Physician. Answer Nursing telephone line messages. Responds appropriately within the scope of Nursing and recommends treatments under the supervision of a physician and/or use of standardized protocols. Maintains open communication with ancillary departments such as pharmacy, sterile processing, EKG and the laboratory to problem- solve issues. Actively participates in all staff meetings. Demonstrates knowledge of and effective use of communication to resolve questions, problems, concerns, and/or issues. Demonstrates courtesy, common sense and follow-through in all telephone communications, maintaining the Telephone Standards as established by the Medical Center staff. ADVOCACY Provides patients and/or their families with the necessary information to make informed decisions utilizing available resources and makes referrals appropriately. Informs patients/family about services available at UCSF and in the community. Job Description Utilizes the nursing process for assessment, planning, implementation, and evaluation to provide safe, therapeutic care. Functions as a member of the interdisciplinary and nursing teams; carries out established nursing procedures and performs related duties as required; performs nursing care for patients with more complex needs with assistance and supervision. Assists with the management of patient care administered by other members of the nursing team. The Pediatric Interventional Cardiology Lab at UCSF Benioff Children’s Hospital consists of the Cath Lab where patients undergo cardiac catheterization procedures. This is done with diagnostic imaging equipment to visualize the arteries and chambers of the heart to treat any abnormality found. The Pediatric Interventional Cardiology lab also consists of the EP Lab where cardiac electrophysiology studies are performed. An EP study is an invasive procedure that tests the electrical conduction system of the heart to assess the electrical activity and conduction pathways of the heart. This position is to support our patients pre and post procedure and to prepare them for transfer to our in-patient area/s or home.
