Responsibilities of Licensed Practical Nurse (LPN): · Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e., physical, cognitive, and socialization factors) in planning the delivery of care. Demonstrates knowledge, skills, and techniques necessary to care for residents with the following needs: therapeutic, neuro and dementia, dialysis, IV therapy, infectious disease, and wound care, if applicable. · Provides nursing care in accordance with Resident Care Policies and Procedures and ensures the safety and well-being of the residents is maintained. · Demonstrates expertise in dispensing medications and treatments by all routes as ordered by the Physician within established time frames. Ensures no medications are left unattended. · Assesses effectiveness and observes for interaction and reaction of medication and treatment. Documents medication and treatment administration in a timely and accurate manner according to Resident Care Policies and Procedures. · Complies with all State Department codes and requirements regarding nursing care and documentation. · Additional duties as assigned to follow the Care Plan. Qualifications of Licensed Practical Nurse (LPN): High school diploma or equivalent required Graduate of an accredited school of nursing Valid PA state licensure as a Licensed Practical Nurse (LPN) or Graduate practical Nurse (GPN). Current AHA health care provider training (BCLS). Demonstrated experience within a skilled nursing facility and ability to provide proof of such experience. Valid PA LPN/GPN License JOB DESCRIPTION: Directs, supervises, and assists nursing assistants and others in providing direct resident care, adjusting assignments as necessary. Promotes a climate conducive to cooperation of all staff to ensure the continuity and quality of resident care. Admits and assesses residents including the initial care plan and all admission assessments. Administers treatments, medications and diet as prescribed; ensures medications/ treatments have appropriate diagnosi Interacts/Communicates well with co-workers, physicians, residents, families and visitors. Participates in discharge planning with physician, and other disciplines, according to resident’s needs. Accepts assignments as “Charge Nurse” when indicated by RN supervisor. Practices nursing according to nursing theory and current standards of practice. Attends scheduled in services/ meetings. Follow s standards of practice and infection control standards for skin treatments; observes residents for any skin changes. Checks narcotic count and reconciliation sheets and signs in logbook at beginning and end of shift. Completes all required resident charting every shift. (i.e.: Medicare documentation/ alert charting) Educates, directs, assists in resident nursing care Makes rounds, checking residents and resident rooms for cleanliness; infection control issues and ensuring residents are cared for properly. Completes 24-hour report and gives thorough report to oncoming nurse. (including any calls that have been placed or need to be placed to physicians, RP’s, etc.) Observes and documents changes in residents’ condition and reports changes to RN supervisor; residents’ physician and resident’s RP if needed. Supervises and assists in bowel and bladder training program for residents, as needed. Able to organize work and prioritize. Receives medications and medical supplies daily and checks accuracy of orders received; logs narcotics appropriately; notifies RN supervisor of any discrepancies as soon as possible. Communicates with direct care staff with all resident care concerns at the beginning and throughout the shift. Answers families’ and visitor’s questions as necessary. Is responsible for emergency equipment and general nursing supplies; restocking medication/treatment carts and any other supplies used. Participates in care plan meetings when asked. Makes rounds with physicians. Reports all accidents and incidents to RN supervisor and documents on resident chart and on incident reports; informs doctor and family. Transcribes physician’s orders as per p/p; ensures all orders are written correctly and have appropriate dx. Monitors hydration of residents, including accurate I & O. Notifies dietician and MD if not meeting fluid needs or exceeding needs and documents same in residents’ chart. Makes rounds during mealtime to see that each resident has received help as needed, assists in hand feeding, assures that sufficient fluids, special feedings and nourishments are administered as needed. Completes assignments timely, i.e.: quarterly evaluations Coordinates activities with ancillary departments to ensure that treatments plans are followed and/or revised as needed. (i.e.: therapy) Utilizes equipment properly and ensures direct care staff does same; communicates to RN supervisor if education is needed. Monitors weight of all residents, utilizing protocol if there is a significant weight loss. Ensure plan of care is being followed. Cooperates with all staff; is respectful to coworkers. Practices safe working habits. Answers call l lights promptly. This client requires full assistance with activities of daily living, including hands-on transfer assistance, and is considered a significant fall risk. We need a caregiver who understands that fall prevention requires constant awareness, close supervision, careful assistance, and proactive safety measures throughout the entire shift. The selected caregiver must be available to work all three scheduled days Monday, Wednesday, and Friday from 12:00 PM to 5:30 PM. Consistent attendance is essential because the client depends on a stable routine and familiar caregivers. The client also has a small dog in the home. Applicants must be comfortable entering and working in a home with a small household pet. At Helping Hearts, caregiving is more than a job, it is a calling. We are looking for someone who will treat the client with dignity, patience, compassion, and respect while remaining actively engaged and focused on the client’s safety. Primary Responsibilities Full Assistance with Activities of Daily Living The caregiver will provide respectful, hands-on assistance with the client’s daily personal-care needs as outlined in the client’s care plan. Applicants without this experience must be willing and able to complete all required hands-on training and demonstrate safe use of the equipment before independently performing mechanical-lift transfers. During every transfer, the caregiver must: Explain the transfer process to the client Make sure the client is positioned correctly Properly position the wheelchair or commode Lock the wheelchair brakes before beginning Move footrests, cords, furniture, and other obstacles out of the way Clear and inspect the transfer area Use proper body mechanics Use approved transfer equipment when required Closely monitor the client’s balance and comfort Move slowly and avoid rushing or pulling the client Stop immediately if the transfer becomes unsafe Transfers must never be rushed or attempted in an unsafe manner. The caregiver must follow the client’s care plan and use any required gait belt, Hoyer lift, sit-to-stand lift, or other approved transfer device only after receiving proper training and authorization. If a transfer requires two caregivers or cannot be completed safely, the caregiver must not attempt it alone. The caregiver must keep the client safe and contact the Helping Hearts office immediately for assistance and further direction. Applicants must be able to perform the essential transfer responsibilities of this position safely and consistently, with or without reasonable accommodation. Proactive Fall Prevention and Safety Monitoring Because this client is at significant risk for falls, the caregiver must remain alert, attentive, engaged, and safety-focused throughout the entire shift. This is not a passive companion assignment or a position where the caregiver can sit nearby without actively observing and assisting the client. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CONSUMER SERVICE • Presents professional image to consumers through dress, behavior and speech. • Adheres to Company Standards for resolving consumer concerns. • Ensures that all consumer/resident rights are protected.
