Under the general direction of the Heart and Vascular Administration , the Clinical Practice Manager is responsible for administrative and patient care activities of the ambulatory physician practice at the Duchossois Center for Advanced Medicine (DCAM). The nurse manager oversees day-to-day operations, including scheduling, planning, organizing, and directing all activities related to patient-centered care in the practice, and handles an array of administrative duties, including recruitment and hiring; supervision; purchasing; inventory; facility, safety and equipment management. Responsible for preparing for and participating in various inspections, including the JCAHO accreditation process. The incumbent will focus on the patient and family experience, quality and safety, coordination of care across the continuum, use of technological tools and solutions including EPIC, metrics reporting, customer service, and oversees the budget, billing and related revenue cycle services. Essential Job Functions Day to day management of practice operations, including Establishes staff schedule and daily assignments. Responsible for time and attendance and monitoring overtime. Oversight of practice specific issues with IS, facilities, EVS, supplies, etc. Ensures compliance with regulatory agencies (i.e. JCAHO, IDPH, CAP/CLIA). Collaborates with providers and staff to ensure efficient patient throughput. Actively monitors performance metrics and daily reports to make real time adjustments. Ensures patients and family’s needs are met. Participates in service recovery at the point of issue. Participates in the recruitment and selection of staff. Identifies recruitment needs and necessary staffing levels. Assists staff members in identification of learning needs and promotes staff participation in professional and institutional activities. Participates in and promotes educational opportunities for staff. Encourages staff participation in designing and attending in-service classes. Participates in periodic review of sectional activities needs assessment and development and prioritization of goals and objectives. Assists in the development and review of policies, procedures, and standards. Confers with physicians and supervisors concerning the care and treatment of individual patients. Assesses, plans, implements and evaluates patient care on a daily basis. Provides orientation and training of staff members in carrying out physicians' orders, seeing to the needs of patients, and developing approved nursing practices. Collaborates with medical director on quality assurance issues. Collaborates with medical director in collecting data for staff meetings, research, reports and evaluation. Promotes safety of patients and personnel by compliance with institution and regulatory agency standards and policies. Defines and organizes quality improvement initiatives. Assists in providing patient care and obtaining necessary documentation for a wide variety of research protocols. Participates in budget development, making recommendations regarding staffing, equipment, and supply issues. Monitors operating budget for labor and non-labor expenses. Promotes cost containment programs. Acts as a role model to promote professional growth and clinical competence. Participates as appropriate in educational programs designed to promote professional growth and development, including planning, teaching and coordination of education programs. Knowledge of and delivers age-appropriate care related to the physical and psychological needs of the resident as per plan of care. Provides assistance with basic health care needs including daily living activities that may include, but are not limited to, bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psycho-social support and other personal care to assigned resident. May assist the resident in transferring, re-positioning, and walking using correct and appropriate transfer techniques and equipment and provides range of motion and passive exercises. Completes tasks related to food preparation and dining, while creating a positive dining experience for the resident. May assist residents with meals and snacks; provide water and supplements as appropriate. Administers prescribed medications as delegated by a licensed nurse and within their scope of practice as defined by state regulations. Documents the outcomes related to resident care, medication administration, behavior, activity, and dietary. Communicates changing condition, medication follow-up and care related concerns/responses to the charge nurse. Collects non-invasive body fluid specimens and gathers vital signs. Provides housekeeping and laundry services to assist residents with keeping living areas clean and operational. Provides assistance and documentation with wellness and recreational programs and encourages residents to participate. May participate in departmental/organization committees and task forces as needed. Qualifications High school graduate or general equivalency diploma (GED) preferred, but not required. Minimum age of 18 preferred. Prior clinical or nursing assistant experience in long-term care preferred. Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year. A minimum of 12 hours continuing education are required annually based on state requirements. Maintains all department specific required certifications. When applicable, may need a valid driver's license and auto insurance. Iowa: Require Certified Nursing Assistant (CNA) certification with the State Board of Nursing. CNA certification must be maintained with the State Board of Nursing. Successful completion of a state-approved Certified Medication Aide/Assistant course once CNA certification is obtained. Minnesota: Must have nursing assistant certification, have completed the 75-hour course and be registered with the Minnesota Nursing Assistant Registry. Successful completion of a state-approved Trained Medication Assistant course. North Dakota: Require Certified Nursing Assistant (CNA) certification with the State Board of Nursing. CNA certification must be maintained with the State Board of Nursing. Successful completion of a state-approved Certified Medication Aide/Assistant course once CNA certification is obtained. If working in an Assisted Living or Basic Care facility, Medication Assistant I certification required. If working in a skilled nursing facility, Medication Assistant II certification is required. South Dakota: Must successfully complete the state approved Certified Medication Aide (CMA) course. CMA certification must be maintained with the State Board of Nursing. For all Good Samaritan and Sanford nursing facilities: Must have the state required certified/trained medication aide certification (CMA or TMA). Active certified nursing assistant registration in state of practice, or ability to obtain within 120 days, is required.
