The goals are the provision of quality health care, decreased fragmentation of care, enhancement of a patient’s self-care and quality of life and cost containment. Incumbent also participates in weekend/holiday rotating on-call schedule to provide medical nutrition therapy for inpatients, with and without diabetes. Qualifications NECESSARY QUALIFICATIONS Education: Must have Bachelor’s Degree in Dietetics or for graduates 2023 and after must have a Master’s Degree in Nutrition, Public Health or related field. Experience: Must have a minimum of 6 months experience as a clinical dietitian in an inpatient or outpatient setting with experience in diabetes self-management. Certification: Must be a current Registered Dietitian in good standing with the Commission on Dietetic Registration (CDR) Must have a registered RD licensure from any state, if none, must obtain within (1) one monthof hire. Provides appropriate level and frequency of care for patients and their families as identified in the individual assessment. Provides diabetes self-management education in both individual and group settings utilizing the IHS “Balancing Your Life with Diabetes” and Association of Diabetes Care & Education Specialist (ADCES) curricula, using ADCES-7 framework. Apply theoretical approaches such as motivational interviewing, stages of change theory, and empowerment theory to facilitate behavior change. Effectively communicates with patients and families in a culturally sensitive manner. Utilize a case management model to follow a specific sub-population of diabetes patients, e.g. newly diagnosed, adolescents, pregnant patients, high-risk patients, etc. Ensure patients receive recommended services and appropriate follow-up. Provides consultant services to the TCRHCC Dietary Department as needed, such as tray-line monitoring, menu updates, and in-service training. Provides inpatient medical nutrition therapy to patients with and without diabetes on a rotating weekend/holiday on-call schedule. Maintains current licensure, job-related competencies, and knowledge of evidence-based guidelines through active participation in continuing education, independent study, seminars, in-services, and dialogue with colleagues. Ensure proper PPE is worn at all times while on duty including but not limited to, face mask, gloves, gown, isolation gown, NIOSH-approved N95 filtering facepiece respirator or higher, if available), and eye or face shield. Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly. Develops, maintains, and implements nursing policies and procedures that conform to current standards of nursing practice, facility philosophy and operational policies while maintaining compliance with state and federal laws and regulations. 2. Communicates and interprets policies and procedures to nursing staff, and monitors staff practices and implementation. 3. Participates in all admission decisions and may visit prospective residents before admission and/or designate a representative to screen prospective residents prior to admission. 4. Participates in daily or weekly management team meetings to discuss resident status, census changes, personnel, or resident complaints or concerns. 5. Evaluates the work performance of all nursing personnel, assists in the determination of wage increases and implements discipline according to operational policies. 6. Ensures delivery of compassionate quality care and nursing supervision as evidenced by adequate services and staff coverage on unit, absence of odors, general cleanliness, prevention of pressure wounds and apparent maintenance of optimal resident functions. 7. Demonstrates knowledge of and application of Key Clinical Quality Indicators and proactively monitors and implements systems to achieve and/or surpass company thresholds. 8. Helps the Administrator prepare staff for inspection surveys, instructing staff on matters of conduct and disclosure, being interviewed by inspectors, immediate corrections of problems noted by surveyors, etc. Reviews and reinforces important standards previously cited. 24. Participates in the preparation of the Plan of Correction response to an inspection survey and implements any follow-up QA required for any nursing allegations. 25. Communicates directly with residents, families, medical staff, nursing staff, interdisciplinary team members and Department Heads to coordinate care and services, promote participation in care plans and maintain a high quality of care and life for residents. 26. Promotes customer service and hospitality and responds to and adequately resolves complaints or concerns from residents or families about nursing services. 27. Monitors facility incidents and complaints daily to identify those defined as unusual occurrences by State policy and promptly reports such occurrences to Administrator for appropriate action. 28. Monitors complaint reports daily for allegations of potential abuse or neglect, or the loss or misappropriation of resident property and participates in these investigations. 29. Promotes compliance with accident prevention procedures, safety rules and safe work practices to prevent employee injury and illness and control worker’s compensation costs. 30. Assures staff is trained in fire, disaster and other emergency procedures. Evaluates staff performance during drills. 31. Interacts courteously with residents, family members, employees, visitors, vendors, business associates and representatives of government agencies. 32.
