Our Inpatient Unit on the Mitchell Campus in Cheektowaga, NY, provides a relaxing home-like setting for select patients and their families. Our unique unit is only offered to patients whose pain or symptoms cannot be managed at home or are approaching their final stage. Hospice patients can also be admitted to the unit for short-term respite care. A Home Health Aide assists in providing quality patient care by performing assigned duties, health care tasks, and specified patient care activities according to the policies and procedures of Hospice Buffalo, Inc. and under the supervision of a licensed practical nurse and/or registered nurse. Assists in establishing and maintaining good interpersonal relationships with hospice patients, families, health care personnel, and the entire hospice staff and volunteers. Responsibilities : Responsible for assisting patients with and per plan of care, the following: Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc. Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency) Safe transfer and ambulation techniques Exercise according to the plan of care Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency) Patient’s laundry Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.) Observation and teaching of home safety practices Recognition of an emergency situation and knowledge of emergency management procedures Must follow patients’ plan of care and complete all tasks assigned Communicates in a constant, clear, and effective way with the client, family/caregiver and office Observes and documents patient condition and reports any changes, to the assigned nurse supervisor Requirements : Meets all training requirements in accordance with state and federal laws when applicable Ability to read, follow written instructions and document care given Possess and maintains current CPR certification in accordance with state regulation PPD or chest x-ray if applicable Successful completion of Maxim screening examinations Participation and completion of Maxim’s competency program Must meet all federal, state, and local requirements The Lakes benefits : part-time position.
