HOW YOU'LL WORK: You'll provide skilled nursing care in a patient's home as directed by the attending physician. You will work autonomously, so you need to be efficient in managing your time. Territory: Hebron, Newark, Fresno, Warsaw, Nashport, Millersport, Coshocton, Dresden areas SUMMARY The Registered Nurse Case Manager (RNCM) assumes ultimate accountability and leadership for the assessment of the patient condition and plan of care.
