LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home. Enhances the abundant life of participants by providing assistance through performance of routine direct and indirect participant activities according to established policies and procedures. Maintains participant rights, privacy, and confidentiality. Promotes participant acceptance of care plans and responds promptly to overall participant needs. Qualifications Community Care Coordinator - C.N.A. or LPN requirements include: High school diploma or general education degree (GED); or 1 year related experience and/or training; or equivalent combination of education and experience. LPN or Current Pennsylvania State Nurse Aide Certification; Must be listed on state nurse aide registry and without allegations. Must have one (1) year of geriatric related experience; Working knowledge of computers, Microsoft Word and Excel required; Excellent interpersonal skills, team-oriented, knowledge of medical terminology, computer literate. Must have and present current valid driver's license. CPR and Certified First Aide required. Caregiver Job Duties Our caregivers support their clients in activities of daily living such as (but not limited to): Household chores (cleaning, laundry, dishes, etc.) Cooking and/or serving meals Helping clients bathe, dress, and groom Providing companionship through daily activities and hobbies Driving clients (as needed) to the store or appointments Monitoring and reporting on their condition Additional Information Our hiring process is fast and simple! Once you have completed onboarding, orientation, and training, you can start working with clients! Washington only – If you do not have a current HCA or CNA license, you will be required to enroll in our HCA training program we offer (due to state requirements). You will be able to complete the training while working and will have 120 days to complete it. Oregon only – If you have not previously completed the state-required 8-hour Home Care Pulse caregiver training, you will be required to complete the training along with the nurse-led medication training before you can work with clients. You will be paid to complete both trainings! Requirements 18 years+ Ability to lift a minimum of 10lbs and up to 50lb rarely.
