Possess a valid driver’s license and insured automobile Responsibilities of the Certified Home Health Aide- Certified Home Health Aides help patients in a variety of ways which may include taking their vital sign or giving them medication under a nurses direction. They must record and report this data as medically necessary. Certified Home Health Aides also can help patients with their daily functions, such as dressing or taking a bath. They can assist with planning for doctor's appointments, other meetings and may organize transportation for them. As a Certified Home Health Aide you will be able to do facility work, hourly as well as live in. Job Title/Position : Hospice Weekend Visit Nurse (RN) Reports To : Clinical Director JOB DESCRIPTION SUMMARY : The Hospice Weekend Visit Nurse (RN) is responsible for filling in for RN Case Managers, On-Call RNs, and Admission RNs experiencing an overage of visits per day during the weekend. The Hospice Weekend Visit Nurse (RN) will provide clinical and psychosocial interventions for patients and their families, skilled pain and symptom management and collaboration with providers within and outside of the hospice team to maximize positive patient outcomes.
