PRIMARY RESPONSIBILITIES Assist clients with personal hygiene (bathing, grooming, toileting, etc.) Support activities of daily living (feeding, mobility, positioning, etc.) Promote client safety and comfort by following care plans and proper procedures Maintain confidentiality and uphold agency standards and policies Document care provided and client responses in a timely manner Provide companionship and emotional support to clients and families Work collaboratively with families, supervisors, and care team members Perform all other duties as assigned QUALIFICATIONS High school diploma or GED (preferred) At least 2 years of related client care experience (preferred) CNA/HHA/Direct Care Worker certification (preferred) Valid driver’s license and reliable, insured transportation required Strong communication and interpersonal skills Must pass background and reference checks SHIFT & LOCATION Every other weekend (Saturday and Sunday) Every Saturday or Every Sunday Service areas: Montgomery County (King of Prussia | Hatfield | Ambler) PHYSICAL REQUIREMENTS Ability to lift, carry, push, or pull up to 50 lbs. as needed ABOUT THE COMPANY Faithful Home Care Solutions is a licensed, Medicaid-certified, non-medical home care agency based in Glenside, Pennsylvania. What you will do as a Registered Nurse with Shared Services Home Health Care : Not only will you be growing in your career, but you will also be gaining another family. Regularly re-evaluates patient nursing needs. Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Provides health care instructions to the patient as appropriate per assessment and plan of care. Visit clients routinely in order to observe and evaluate each clients’ physical and emotional status. Maintain the confidentiality of all client care information. Monitor nursing care to ensure that all clients are treated fairly, and with kindness, dignity, and respect.
