Administers prescribed medications and treatments as permitted by law. Assesses all medicines a client may be taking to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, contraindicated medications and reports any problems to the physician. Re-evaluates client’s nursing needs and reports findings to Case Manager. Counsels and instructs the client and family in meeting nursing and related needs. Prepares clinical and progress notes. Submits all documentation in a timely manner in accordance with agency policy. Documentation on patients chart correlates with nursing care plan. Communicates frequently and works closely with other home care team members. One year of experience in home health experience is preferred. Provides quality in-home care and infusions as defined within the scope of practice by the Nurse Practice Act. Administration and self-administration teaching of specialty pharmacy infusion therapies and injectables to patients with rare, chronic, and acute diseases in the home setting, infusion suites, or physicians' offices. Coordinates the planning for delivery of nursing care with the Director of Nursing and administers appropriate clinical decisions in the delivery of patient care. Proactively updates client/patients’ POC, health changes, and other related incidents. Demonstrates knowledge of age-specific differences in the patient population, including physiological and developmental differences unique to each group. Ensures safe care to patients; adheres to all policies, procedures, and standards, including time management, supply management, productivity, and quality of service Willingness to travel within the assigned geographic region. Flexibility to work alternate shifts on short notice and can be on call for field visits as determined by business needs. Administers CPR and other emergency procedures, as necessary. Qualifications: Licensed as a Registered Nurse through the State Board of Nursing is required. Home health experience preferred – Can perform assessments, wound care, medication management, and infusions. Minimum of two years medical, surgical, or critical care/ER experience preferred.
