Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of 'direct' and 'indirect' patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. This is performed in accordance with Family Connects International model and OCHIDS documentation system. Essential Functions: Provides services in accordance with the plan of care. Makes the initial evaluation visit and completes documentation and follow up as dictated by the Family Connects model. Refers patients to the appropriate resources as needed. Provides those services requiring substantial specialized nursing skills. Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy. Coordinates services. Informs patient’s PCP of changes in the condition and needs of the patient as appropriate. Counsels the patient and family/significant others in meeting nursing and related needs. Understands and adheres to established Agency policies and procedures. Completes and submits OCHIDS documentation in accordance with Agency defined time frames. Positively represents the department and Fisher-Titus in community interactions.
