Hospice Aide (CNA)

Position Summary The Hospice Aide provides compassionate, hands-on personal care to terminally ill patients in various settings. The aide supports patient comfort, dignity, and safety while adhering to the established plan of care. The role involves direct care tasks, assisting with mobility, and supporting family caregivers, with flexibility to work weekends, holidays, and across different care sites as needed. Primary Location: Community Hospice Bailey Center for Caring located at UF Health in St. Augustine, FL Shift: 7 p.m. - 7 a.m. - 36 hours a week Sign on Bonus: $1000 Essential Functions Provide personal care services in accordance with the plan of care and hospice regulations. Assist with patient ambulation and exercise routines. Perform basic procedures as directed by licensed clinicians. Observe and report changes in the patient’s condition to a licensed nurse. Complete timely and accurate documentation per policy. Educate family or caregiver on proper patient care techniques. Maintain confidentiality of all patient information. Participate in interdisciplinary team meetings and other required sessions. Complete a minimum of 12 hours of job-specific training annually. Perform other duties as assigned.
