Be the Best Hospice After Hours Admission RN You Can Be If you meet these qualifications, we would love to meet you: Current licensure as a Registered Nurse in the applicable state is required, with an associate degree preferred. CPR certification is recommended (required for direct patient care in Georgia), along with prior experience in hospice, home health, medical-surgical, or population-specific nursing such as geriatrics or pediatrics. Must demonstrate a willingness to participate in an on-call rotation and maintain reliable transportation, valid driver’s license, and state-required liability auto insurance. Strong communication and organizational skills are essential, along with proficiency in Microsoft Office Suite and EMR systems. The ability to collaborate effectively within an interdisciplinary team is required. This role involves regular physical activity, including lifting patients over 200 lbs with assistance, transporting supplies and equipment up to 20 lbs for distances of 500 feet, and frequent standing, walking, sitting, stooping, and reaching. Responsibilities: Conducts comprehensive nursing assessments and collaborates with patients, families, physicians, and the Interdisciplinary Group (IDG) to develop, implement, and evaluate individualized plans of care in alignment with regulatory requirements and the Case Management Recipe. Maintains timely and accurate documentation, ensures care plans reflect current patient status, and communicates effectively with the care team to coordinate optimal use of resources, especially in complex or high-risk situations. Provides education and emotional support to patients and caregivers based on individual needs, and actively participates in on-call rotations, admissions, and care transitions as directed. If serving as a Care Manager, coordinates all aspects of patient care including scheduling, medication management, plan of care oversight, IDG collaboration, and supervision of LPNs, LVNs, and Hospice Aides; participates in IDG meetings and patient/family conferences. If serving in an On-Call capacity, responds to triage calls, performs visits and assessments, coordinates care with the Administrator On Call, and may attend IDG meetings and family conferences; also participates in training, committees, and other duties as assigned within licensure scope. Evaluates nursing, medical, social and educational needs on assigned patients. Writes progress notes for each visit documenting pertinent information regarding patient progress and treatment regime. Writes orders appropriate to the patient's care requirements in compliance with existing contract. Establishes medical diagnoses in compliance with contract. Refers to the physician or designee those problems which require consultation and documents same on progress note. Performs technical functions as covered by the job description and delegation agreement submitted to BPQA and/or MBON, approved NP agreement and in accordance with hospital policy. In an environment of continuous quality improvement, t he Registered Nurse is responsible for rendering professional nursing care and directs and guides patient teaching and activities of ancillary personnel . Exhibits the MHS Standards of Excellence and exercises strict confidentiality at all times. Job Functions: Participates in bedside shift report/handoff. Performs hourly rounding. Obtains required patient assessments and reassessments per nursing policy. Develops a plan of care and teaching plan based on assessed needs of patient and family. Revises as needed per hospital policy. Coordinates timely, adequate teaching to patient/family/significant other in preparation for discharge and documents appropriately. Implements and monitors nursing interventions. Consistently documents assessments, interventions, and outcomes in Meditech within required timeframe. Participates in interdisciplinary rounding and collaborates with team members to identify patient needs. Recognizes changes in patient conditions and communicates with physician using SBAR. Effectively manages patient care and delegates appropriately according to the needs of the patient and the skill of the caregiver. Utilizes appropriate interpersonal communication which facilitates productive relationships with other team members. Facilitates a positive and caring relationship with others by utilizing trust and mutual respect. Establishes a therapeutic relationship between patient, family and self, based on empathy and trust. Promotes a positive patient experience. Seeks help from appropriate resources as needed. Maintains current knowledge of and practices within the scope of hospital policies. Clarifies any unclear communication between members of the healthcare team. Uses proper channels of authority in communicating information. Collects Data related to physical, psychological and psychosocial health status. Develops a plan of care and teaching plan based on assessed needs of patient and family. Revises same as diagnosis and needs dictate. As part of the nursing process, performs admission, handoff, transfer and discharge procedures. Coordinates with physician’s plan of care, the admission assessment and discharge needs. Maintains compliance in RQI. Assumes all other duties and responsibilities as necessary. Charge Nurse: In addition to above essential functions, the charge nurse will also : Complete daily nurse leader rounds. Perform shift checks as assigned. Assists with completion of chart audits for nursing documentation as required. Complete staff assignments taking into account the skill level and acuity of patient assignment. Manages throughput on the nursing unit and make changes to assignments as needed. Reports to the nursing unit manager any problems in regards to performance, safety, staffing, behavior, etc. Assists with conflict resolution in the absence of the nursing manager in collaboration with the house supervisor. Role models positive behaviors with staff including integrity, fairness and consistency. Agent for change for the department. Assigned and assist as super user for completion of incident report. Minimum Education/Experience Required: Current licensure in the State of Ohio or if working in WV will need a WV license as a Registered Nurse. BLS required upon hire. ACLS within 1 year of employment required. Special Knowledge, Skills, Training: Verbal, clerical, numerical, and manual dexterity, color vision and near visual acuity, proper lifting techniques and body mechanics.
