What You'll Do as a Hospice RN / RN Case Manager: Be the primary point of contact for patients and families, guiding them through every step of their hospice journey. Deliver and document skilled, hands-on nursing care based on each patient’s individualized Plan of Care—in their home, assisted living, or wherever they call home. Perform initial and ongoing assessments to monitor patient condition and adjust care as needed. Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, chaplains, and others—to build and refine personalized care plans. Provide emotional, physical, and spiritual support not just to the patient, but to their loved ones as well—offering education, comfort, and bereavement guidance. Teach caregivers how to care for their loved one safely and confidently, offering both practical training and emotional reassurance. Communicate important updates in patient condition to the care team, ensuring timely and appropriate interventions. Maintain accurate, up-to-date clinical documentation and follow infection control and compliance standards. Support the organization's quality improvement and scheduling initiatives to ensure care is consistent, compassionate, and efficient. Salary Range – $70,410-82,785/year Essential Functions Job Responsibilities Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments/screenings and provide education to patients, family members and caregivers as appropriate. Deliver skilled nursing interventions and symptom management aligned with goals of care. Develop and update individualized plans of care including patient visit frequency to meet individual needs and risk identification. Coordinate care across interdisciplinary teams including consultation with physicians and nurse partitioners. Provide patient and caregiver education regarding disease process, self-care techniques, end-of-life care, medications, nutrition and dietary needs. Communicate change in condition, or signs and symptoms of decline to appropriate provider (e.g., NP, PCP, Hospice Physician). Support the continuity of care by recognizing and supporting home health, palliative and hospice referrals as appropriate and recognizing and supporting transitions in levels of care. Document timely clinical records on all patients including screening/assessment findings, physician orders, progress notes, and care plans. Maintain effective working relationships with IDG, and involve team when patient's care plan warrants team participation to achieve desired outcome. Participate in on-call and patient support activities as applicable, conducting on-call services in a clinically competent and responsive manner. The requirements listed below are representative of the knowledge, skill, and/or ability required. Education: Registered Nurse Experience: A minimum of one (1) year experience as an RN, either in acute care hospital, oncology, long-term care, hospice or home health. School internships and clinical hours may be considered as experience in individual cases. Hospice or palliative care experience preferred Licenses, Certifications and/or Registration: Current license to practice nursing in the state of requested employment CPR Certification Current automobile insurance and valid driver’s license Specialized Knowledge and Skills: Knowledge of terminally ill patients and their families along with understanding of hospice; ability to apply knowledge of the special needs of hospice patient and families; knowledge of roles of all disciplines providing hospice services; excellent patient assessment skills; knowledge of community resources; good oral and written communication skills; documentation and program management; knowledge of general nursing practice; thorough knowledge of managed care principles, regulatory guidelines (i.e., Medicare, Medicaid, ACHC, and human resource management principles). Working Conditions: Patient’s homes and/or institutional setting and automobile. Potential exposure to infections, communicable diseases, odors, blood, excreta and hazardous materials. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of chapters 246- 335 WAC and 70.127 RCW to the Washington Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of RCW 74.34.020 or 26.44.030 to the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. Educates/counsels patients, families, and/or caregivers as to preventative care, medical problems, psychological problems, and spiritual problems in conjunction with the interdisciplinary team to meet the total needs of patients. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to identify the goals of care and meet the care needs of the patient and family/caregiver. 1 Serves as a nursing resource for consultation and educations to members of the interdisciplinary team and other healthcare practitioners in the community. 1 Establishes, builds and nurtures relationships with staff and community referral sources to facilitate program growth. Additional Duties 1. Maintains knowledge of and compliance with current Medicare/Medicaid, state/federal rules and regulations for hospice services 2. Ensures compliance with the Medicare conditions of participation and other state regulations govern the provision of healthcare. 3. Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state and organizational policies. 1. Participates in organizational monitoring of the quality of medical services and quality improvement initiatives. 2. Assumes responsibility for personal growth. Develops, maintains and upgrades professional knowledge and practice skills through attendance at seminars, conferences and participation in continuing education and in-service classes. 3. Fulfills the obligation of requested and/or accepted assignments. 4. Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues. Position Qualifications 1. Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program. 2. Has a current DEA license 3. Current nursing licensure in State and CPR certification. 4. Master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience. 5. Accreditation from an approved certifying body for advanced practice nursing as required by State. 6. Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics) 7. Excellent observation, verbal and written communication skill, problem solving skills, mathematical skills; nursing skills per competency checklist. 8. Prolonged or considerable walking or standing. Able to lift, position and / or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills. 9. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
