Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient. Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry. Feeds and assists with meals. Obtains temperature, pulse and respirations as supervised by the Hospice Nurse. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies. Interdisciplinary Collaboration Collaborate closely with physicians, social workers, chaplains, aides, and other members of the care team. Communicate changes in patient condition and coordinate appropriate interventions. Participate in interdisciplinary team meetings and care planning discussions. Quality & Compliance Adhere to hospice, state, federal, and regulatory requirements. Follow infection prevention and control practices. Participate in quality improvement and performance initiatives. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5.
