DUTIES: 1. Assess and evaluates students health pertinent family history and home factors, counsel students, parents and school personnel about health problems and issues. 2. Documents pertinent information in student records according to the Standards of Practice 8. Provides emergency care for all students on school premises in the event of injury or sudden illness 9. Medication administration to students with parent/guardian and physician consent 10. Perform site mandated screenings in vision, hearing, scoliosis, TB testing and other health related activities as deemed appropriate by state and city departments of public health 11. Provide information about health and disease prevention in cooperation with health education programs as requested by administrators 12. Participates in school health related activities as deemed appropriate for school nurse by principal/supervisor 13. Acts as a resource in health education to students and families 14. Initiate, promote, maintain and implement programs for the recognition, prevent and control of disease, including giving immunizations. 15. Participate in 766 Core evaluation team by coordinating medical evaluation, providing medical data and appropriate interpretations, provide sociological assessment and early childhood screenings as requested. 16. Collaborate with staff in school and health centers. 17. Sport Physical clearance coverage as needed. QUALIFICATIONS: Must have a current valid, license to practice as a Registered Nurse in Massachusetts. Have three years of responsible professional experience in school nursing or a related field preferred. Current certification in cardio-pulmonary resuscitation Must be DOE certified or certifiable by the Department of Education in educator category, Nurse, All Levels. MDS Coordinator The primary role of the MDS Coordinator is to coordinate and guide the overall process and tracking of all Medicare/Medicaid case-mix documents, and care assessments in order to assure appropriate reimbursement for services. Assists the Supervisor, Recreational Activities and other activities staff with individual and group programs by helping to plan individual treatment programs, increasing participants’ motivation to participate, assisting participants to participate when needed, and assisting with evaluation of program effectiveness. Assists registered therapists and certified therapy assistants with treatments and participant-specific activities which are ordered for each participant and assigned by the registered therapist including, but not limited to, positioning, transfers, ambulation, and exercises. Maintains a clean and safe working and/or living environment in the PACE center and/or participants’ homes. Assists with meal and snack preparation, serving, feeding as needed, and clean-up. Uses safe techniques in all interactions with participants at the PACE Center and in participants’ homes. Provides accurate and timely documentation in the EMR and other systems as required by SE policies and procedures and/or as assigned by supervisor. Qualifications Education: Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable, but we will accept equivalent work experience in lieu of certificate. Certification: Certification as a Home Health Aide or Nursing Assistant is desired. Reliable Transportation Required. Experience: One year of experience working with a frail and/or older adult population, preferably in a PACE program. If under one year, must complete Summit Eldercare training program for health aides prior to working with participants independently. Must complete a standardized set of Summit Eldercare competencies for Health Aides on an annual basis.
