The Optum Serve Physician Assistant or Nurse Practitioner provides clinically competent delivery of quality virtual medical care services required during Optum Serve operations. The PA or NP is responsible for providing clinically competent medical care services, within their scope of practice. The PA or NP working remotely on behalf of Optum Serve is responsible for safe patient care and ensuring adherence to all Optum Serve Policies and Procedures. All PA or NP shall meet the qualifications for their specialty and perform essentially the same functions, within the scope of acceptable standards for their technical professional discipline, as those required by DoD or government service technical professionals of similar experience and in similar duty assignments. This position works in collaboration with other providers as well as other team members of the healthcare team to complete Virtual health assessments in non-traditional healthcare settings. This provider is responsible for virtual patient triage, assessments, and referrals of all presenting patients to Optum Serve Telehealth. Seeking to hire PAs or NPs who reside or hold licensure in the following states with ability to obtain multiple state licenses per regional assignment: KS, OR, AR, AK, IA, ME, KY, CT, NH, WV, DE, MT, ID, SD, NM, RI, ND, VT, WY, WY, NE Essential Functions Include: This role is entirely remote and requires working in a non-traditional clinical setting. It is also (virtually) customer-facing with high expectations for operational excellence. The noise level is moderate and there is no risk of exposure to infectious diseases and blood-borne pathogens. Hours may vary based on location, patient volume, and business needs. Required Qualifications: Master's Degree from an accredited university in a Physician Assistant Program or Master's Degree from an accredited university in a Nurse Practitioner program Valid and unrestricted professional PA or NP license in the state which you reside Ability to apply for 10+ additional assigned state licensures within 2 weeks of hire date Collaborative physician agreement if applicable based on state scope of practice regulations 1+ years as a PA or NP in a clinical environment including oversight of ancillary and clinical staff and caring for adults Experience conducting assessments on wide variety of acute/chronic conditions Proven experience utilizing electronic health record (EHR) platforms to document, review, and manage patient information Ability to work one or more of the listed schedule requirements including one weekend rotation per month Preferred Qualifications: 2+ years clinical experience in a health care delivery system VA and/or military experience Experience working in a Virtual setting Experience working in a call center environment, handling a high volume of calls and performing repetitive tasks while maintaining quality and productivity standards Familiarity with government forms and documentation Proven leadership experience including the ability to supervise, coach and develop staff using excellent interpersonal and communication skills Has multiple licensures preferably in one or more of the following states with ability to obtain multiple state licenses per regional assignment: KS, OR, AR, AK, IA, ME, KY, CT, NH, WV, DE, MT, ID, SD, NM, RI, ND, VT, WY, WY, NE Excellent written and verbal communication skills Excellent social and therapeutic communications skills Excellent interpersonal and problem-solving skills Excellent computer skills in Microsoft Office and database experience *All employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter Policy Compensation for this specialty generally ranges from $113,000 - $173,500. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. What You'll Do as a Hospice RN / RN Case Manager: Be the primary point of contact for patients and families, guiding them through every step of their hospice journey. Deliver and document skilled, hands-on nursing care based on each patient’s individualized Plan of Care—in their home, assisted living, or wherever they call home. Perform initial and ongoing assessments to monitor patient condition and adjust care as needed. Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, chaplains, and others—to build and refine personalized care plans. Provide emotional, physical, and spiritual support not just to the patient, but to their loved ones as well—offering education, comfort, and bereavement guidance. Deliver skilled nursing interventions and symptom management aligned with goals of care. Develop and update individualized plans of care including patient visit frequency to meet individual needs and risk identification. Coordinate care across interdisciplinary teams including consultation with physicians and nurse partitioners. Provide patient and caregiver education regarding disease process, self-care techniques, end-of-life care, medications, nutrition and dietary needs. Communicate change in condition, or signs and symptoms of decline to appropriate provider (e.g., NP, PCP, Hospice Physician). Support the continuity of care by recognizing and supporting home health, palliative and hospice referrals as appropriate and recognizing and supporting transitions in levels of care. Maintain effective working relationships with IDG, and involve team when patient's care plan warrants team participation to achieve desired outcome. Participate in on-call and patient support activities as applicable, conducting on-call services in a clinically competent and responsive manner. Hospice or palliative care experience preferred Licenses, Certifications and/or Registration: Current license to practice nursing in the state of requested employment CPR Certification Current automobile insurance and valid driver’s license Specialized Knowledge and Skills: Knowledge of terminally ill patients and their families along with understanding of hospice; ability to apply knowledge of the special needs of hospice patient and families; knowledge of roles of all disciplines providing hospice services; excellent patient assessment skills; knowledge of community resources; good oral and written communication skills; documentation and program management; knowledge of general nursing practice; thorough knowledge of managed care principles, regulatory guidelines (i.e., Medicare, Medicaid, ACHC, and human resource management principles). Job Summary and Qualifications The Labor and Delivery unit delivers approximately 350 babies per month and care for 700-900 outpatients per month. Their focus is on high risk labor for women age 14- 55. They have the highest employee engagement scores in the hospital, with very active committees which contribute to the shared governance on the unit. Over 70% of LDR nurses have received their National Nursing Certifications. Additionally this unit has been remodeled and the project was completed in Spring 2023. Responsible for the delivery of patient care through the nursing process of assessment, diagnosis, planning, implementation and evaluation. Responsible for directing and coordinating all nursing care for the department-specific patient population in accordance with the standards of nursing care and practice as defined by Wesley Medical Center and the Kansas Nurse Practice Act. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and achievement of desired patient outcomes. Supports the organization vision and mission. Utilizes knowledge of patient’s age and cultural diversity in the provision of patient care.
