Hospice Aide STNA

JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of chapters 246- 335 WAC and 70.127 RCW to the Washington Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of RCW 74.34.020 or 26.44.030 to the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. JOB LIMITATIONS The hospice aide will not function in any manner viewed as the practice of nursing according to the Nurse Practice Act. Specifically, the hospice aide will not administer medications, take physician's orders or perform procedures requiring the training, knowledge and skill of a licensed nurse, such as sterile techniques. POSITION QUALIFICATIONS 1. Completion of a state approved certified nursing assistant course or equivalent training 2. Current Healthcare Provider CPR (Cardiopulmonary Resuscitation) certification ((may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer) 3. Due to nature of job and working with multiples facilities with multiple covid guidelines must be covid vaccinated. 4. High school diploma or General Equivalency Degree (GED) 5. Ability to read and follow written instructions and document care given. 6. Understands hospice philosophy, comfortable providing specialized care to the terminally ill. 7. Prior experience in hospice, orderly, acute , rehabilitative, or long term care facility. 1 year previous home health/hospice experience preferred 8. Basic computer skills to document in electronic medical records (EMR) 9. At least 18 years of age. 10. Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.

Deliver urgent, in-home, patient care under the direction of a licensed provider throughout the assigned market including initial assessments, routine follow-up visits, and ongoing care coordination. Conduct comprehensive home safety evaluations and document findings during patient visits. Deliver patient care under the direction of a licensed provider by: Obtaining patient histories Reviewing medications and treatment plans Performing physical assessments Identifying and addressing health concerns observed or reported during home visits Perform clinical interventions as appropriate and at the direction of the overseeing provider, including medication administration (PO, SL, IM, IV), wound care, point-of-care testing, phlebotomy, Foley catheter care, IV fluid administration, and other approved nursing procedures. Maintain accurate, timely, and complete patient records, ensuring documentation is updated at each patient encounter. Provide patient and caregiver education as directed by the APP-Nurse Practitioner and interdisciplinary care team. Deliver high-quality, patient-centered care while meeting established clinical, quality, and operational performance standards. Support care transitions, end-of-life discussions, palliative care interventions, and the closure of preventive care and HEDIS quality gaps, including screenings and medication adherence initiatives. Complete and submit all required documentation, reports, and administrative paperwork within established timelines. Participate in Interdisciplinary Team (IDT) meetings, providing updates and insights from patient and family interactions to support care planning. Collaborate closely with APPs, nurses, social workers, behavioral health specialists, and other care team members to develop and implement individualized patient care plans. Attend and actively participate in required training programs, including medication administration, clinical equipment use, new protocols, and ongoing professional development opportunities. Build and maintain positive relationships with local healthcare providers, community organizations, caregivers, and referral sources to support WellBe's presence and reputation within the community. Utilize available internal and external resources to address patient needs, including behavioral health services, community-based programs, and other supportive care resources. Maintain all required active, unrestricted state licensure and certifications. Adhere to professional standards of practice, ethical conduct, patient confidentiality requirements, and organizational policies. Perform additional duties and responsibilities as assigned to support organizational goals and patient care excellence. Scheduled working hours: 8am-5pm EST (tentative) Job Requirements What you bring Educational/Experience Requirements : Active State Licensure required; compact licensure preferred. 3+ years as an RN in a direct, patient-facing care delivery role (required). CPR Certification is required. Prior Telehealth experience (preferred). Home care experience (preferred). Previous experience in hospice, palliative care, or geriatrics strongly preferred; other complex patient health care experience will be considered. Possess a valid driver's license with an acceptable driving record. Requires proof of an active auto insurance policy. Required Skills and Abilities: Ability to conduct in-home setting visits and have previous experience on a telehealth platform Demonstrated role proficiency with Microsoft Office; Excel, Word, G-suite, etc. Proficient with Microsoft 365 software suite, Electronic Health Records, Scheduling Software, and Ring Central applications Role model in courageous teamwork, integrity, ethics, and productivity Excellent verbal, written, presentation, and interpersonal communication skills Strong communication skills, with the ability to effectively interact with patients, families, and remote healthcare professionals. Ability to work independently and make sound decisions while adhering to established protocols and guidelines. Excellent organizational and time management skills to handle administrative tasks and facilitate telehealth consultations. Experience working with culturally diverse and underserved populations. Supervisory Responsibility : This position will not have supervisory responsibility. Travel requireme nts: Travel is required up to 100% locally to deliver patient care. Work Conditions: Ability to lift up to 20lbs. Moving lifting or transferring of patients may involve lifting of up to 50lbs as well as assist with weights of more than 100lbs. Ability to stand for extended periods. Ability to drive to patient locations (ie. home, hospital, SNF, etc) or community resource locations. Fine motor skills, and Visual acuity.
