Job Description Summary: Infusion Nurse Educator is a licensed Registered Nurse responsible for providing clinical education and training to ensure successful transition of patients to an Option Care Health care delivery model. This role is also responsible for providing intermediate direct patient care to patients in the home, or in an alternate infusion suite, to ensure patient safety with continuity and compliance under a physicians plan of care. The Infusion Nurse Educator will also be responsible for partnering with the sales team to safely and effectively transition patients to on to OCH services. Job Description: Evaluate, educate, and train patients, caregivers, and facility staff about how OCH services & products will be facilitated in an alternative site, in-home, or virtually in order to ensure successful transition of patients to an Option Care Health delivery model. Conduct patient assessments and evaluations to determine patient viability to join the OCH care delivery model. Partner with, and coordinate with the OCH sales team to understand clinical transition educational needs, and to develop improvements aimed at increasing patient transition volume. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Option Care’s policy. Coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Demonstrates compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule.
