Provider shall obtain, maintain and participate in the Medicare and Medicaid Programs, workers compensation, other federal and state reimbursement programs, and the payment plan of any commercial insurer, health maintenance organization, preferred provider organization, accountable health plan, and other health benefit program requested by Legacy. 2 years of experience is required. Previous experience in an FQHC clinic setting is preferred Transitional Care Management experience is preferred Value Based Care/Managed Care experience highly desirable Excellent communication skills and customer service skills. Must be able to manage multiple deadlines and prioritize. Demonstrated competency with an EHRS system. Ability to multitask in a fast-paced environment. Above average organizational skills. This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management. Bilingual / Spanish fluency is desirable. Memorial Hermann Endoscopy Center North Loop is a multi-specialty surgery center committed to producing the highest quality work and experience for patients and their families. Job Summary: The Preadmission Nurse (PAT RN) has the responsibility for organizing and directing activities in accordance with nursing standards, and applicable federal, state and local guidelines. PAT RN may assume the following PAT responsibilities: contact, by way of telephone, patients prior to surgery to assess medical and medication history, review for appropriate lab, x-ray, EKG orders to determine where and when these procedures are to be completed and provide preadmission teaching. The RN can expect to care for adult patients, including the older adult through geriatric, and pediatric patients from 4 months through adolescence. The RN may perform laboratory testing required prior to surgery as ordered. Job Duties and Responsibilities: Other duties may be assigned: 1. Plans and prepared for the preadmission of the patient. 2. Meets all required deadlines. Demonstrates discretionary ability in establishing priority. 3. Delivers indirect and direct patient care according to established standards, taking into consideration the age and risk factors of the patient. 4. Possess the knowledge and ability to use the nursing process to assess, plan, implement, and evaluate the care of the patient. 5. Uses nursing judgment in decision-making. 6. Acts as a patient advocate in meeting his/her physical and psychological needs. 7. Performs patient assessment and reassessment immediately upon admission and on a regular basis in accordance with standards of care with documentation of these assessments as a basis for planning each patient’s care. 8. Ensures that all nursing interventions and patients' responses are communicated to responsible practitioners as indicated and that all components of the nursing process are documented in accordance with standards of practice. 9. Integrates and coordinates care across settings and among disciplines. 10. Implements physician’s orders effectively and safely in accordance with standards of practice. 11. Ability to recognize and intervene in stressful situations. 12. Maintains BLS, ACLS and PALS certifications as applicable. 13. Maintains or acquires skills on equipment necessary to complete required responsibilities. 14. Documents all records according to established policies and procedures. 15. Assesses information/documents on AdvantX, maintaining patient confidentiality. 16. Assists with reviewing and updating policies and procedures. 17.
