Ensures patients /families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc. Coordinates with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
