Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Description The practice of nursing as a Licensed Practical Nurse is defined as performing tasks & responsibilities within the scope of supportive & restorative care; medication administration and specialized tasks under the direction of a Registered Professional Nurse of licensed or otherwise legally authorized physician or dentist. Responsibilities A. Clinical/Technical/Service 1. Participates in the application of the nursing process to an assigned group of patients. 2. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse 3. Performs job duties under supervision of a Registered Professional Nurse. 4. Demonstrates a competent level of approved skilled nursing care tasks and procedures using approved technical skills related to vital signs, pulse oximetry, venipuncture, starting standard peripheral IVs only (not midlines or long peripheral lines), urinary catheter insertion/catheter care; and simple dressings, remove/discontinue a peripheral line. 5. Administer IV fluids, provided completion of annual competency IV therapy training. 6. Monitors patients receiving blood and blood products and TPN under the direction of the RN, who administers the transfusion. 7. Makes recommendations in the application of the nursing process. 8. Participates in planning, and administers direct patient care, including activities of daily living (ADL). 9. Performs point of care testing including blood glucose. 10. Performs nursing procedures as defined in the nursing procedure manual. 11. Administers medication as defined in nursing policy. ALL routes except cannot administer medications IV push/bolus (except for a Normal Saline or heparin flush) or via a central line. LPNs do not administer chemotherapy or immunotherapy. 12. LPNs cannot administer first dose of an IV drug. The RN will administer the first dose of an IV drug and assess for side effects, after which the LPN can administer subsequent doses. 13. LPNs working exclusively in chronic outpatient hemodialysis settings may, under the direct supervision of a Registered Professional Nurse (RN) and after completing an approved hemodialysis training and competency program, perform dialysis-related tasks as permitted by the New York State Education Department (NYSED) Office of the Professions and 10 NYCRR §700.4(b). See Section D for more details regarding role of LPNs in the Outpatient Hemodialysis setting. This exception does not permit independent practice or unsupervised access to central venous lines. 14. Assists in patient assessment after the RN has completed the initial patient assessment, including pain management. 15. Carries out medical and nursing orders. Cannot accept verbal orders or carry out RN standing orders. 16. Makes, reports and records observations on assigned patients within the EMR. 17. The training should include a written examination and competency demonstration. A Hemodialysis LPN may: • Administer bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first dose administration which is administered by an RN. The RN will access and care for patient’s initial catheter use. Subsequently, and following the MSHS CVAD care policy, the LPN can: • Identify and set up supplies necessary for accessing a CVAD. • Initiate treatment with a CVAD. • Draw blood specimens from a CVAD. • Perform CVAD dressing change and exit site care. • Monitor and adjust flow rates of central venous catheter access device/line. • Intervene if there is a catheter/blood line disconnection or other complications associated with a CVAD. • Provide CVAD care at the end of dialysis treatment. Description The Medical Assistant II provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Mentors and assists less experienced staff members. Responsibilities Fulfills patient care responsibilities as assigned which may include: performing venipuncture and/or EKGs, checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring and dressing, as well as collecting and processing specimens and point of care testing, preparing for exam, etc. Assists physicians with various procedures, takes vital signs, relays instructions to patients and or families; answers calls and provides pertinent information. Description As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Primary Responsibilities - Participate in pre-admission evaluations, treatment planning, and development of discharge plans. - Prepare, review, and evaluate nursing intervention plans consistent with each individual’s comprehensive treatment plan. - Develop holistic nursing assessments addressing medical, psychological, social, cultural, and developmental needs. - Oversee implementation of nursing care and modify plans as needed. - Coordinate nursing services with interdisciplinary team members. - Conduct in-service training on hygiene, medication administration, and other health-related topics. - Supervise medication administration practicums and monthly sign-offs. - Identify and report communicable diseases and implement prevention measures. - Maintain detailed medical records, including treatments, medications, vital signs, and nursing observations. - Update nursing plans annually or as needed based on developmental/medical changes. - Provide on-call support and flexible shift coverage. - Coordinate with consulting medical providers and ensure integration of recommended treatments. - Maintain inventory standards for medications and medical supplies. - Ensure infection control practices and safe use of medical equipment. - Provide health education to individuals and families. - Participate in quality assurance and risk management. - Provide emergency care and crisis intervention. - Access, evaluate, and monitor medication effects and side effects. - Provide nursing coverage across sites including vacation and triage rotation (after-hours/weekends/holidays). - Perform duties specific to Residential Nursing in IDD: appointment coordination, follow-up on medical recommendations, medication audits, weekly medical meetings, progress documentation, and training of staff. - Perform other duties as assigned. 
Caregiver/Family Advocacy and Support Services enhance the youth’s ability, regardless of disability (developmental, physical, and/or behavioral), to function as part of a family unit and enhances the family’s ability to care for the youth in the home and/or community as well as, provides the youth, family and caregivers with techniques and information so that they can better respond to the needs of the participant. These services are intended to assist the youth and family in understanding and addressing the participant’s needs related to their disabilities. Responsibilities include: Interact and engage with family and youth to offer educational, advocacy, and support resources to develop family’s ability to independently access community services and activities Provide guidance in the principles of youth’s chronic condition or life-threatening illness Train youth and/or the family in methods and behaviors to enable success in the community, self-advocacy and self sufficiency Requirements: Level 1: Requires a high school diploma or GED, with experience working with youth Level 2: Requires a Bachelors degree plus two years of related experience, or Masters degree in education or human services field plus one year of applicable experience Valid NY or NJ Driver’s license preferred. Description The program focuses on recruiting and training the very best candidates, and prepares the new recruits for the high demands of busy practices, and deploying a highly prepared group of staff into the FPA practices. This position will require consistent rotation in various Faculty Practices; flexibility is a must. You will be responsible for the following: Responsibilities Fulfills patient care responsibilities as assigned which may include: checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring, dressing, collecting and processing specimens and point of care testing, preparing for exam, etc. Assists physicians with various procedures, takes vital signs, relays instructions to patients and or families; answers calls and provides pertinent information Fulfills clerical responsibilities as assigned which may include: sending and receiving patient medical records; obtaining lab/x-ray reports, hospital notes, referral information, etc. Completing forms and requisitions as needed, managing charts to ensure information is completed and filed appropriately. These responsibilities may be carried out using the electronic medical record (EMR) system or manually as necessary Enters orders into the electronic medical record (EMR) system or prepares prescription refill requests on behalf of the physician for the review and approval by the ordering physician Inventories, orders or re-stocks medical supplies as applicable Provides set-up of examination room and instruments in accordance with proper sterilization techniques established by hospital and infection control policies Maintains logging system as required for refrigeration of medications, point of care testing, sterilization process, crash cart, point of care testing or others as needed Checks expiration dates on all medications and medical supplies disposing appropriately as per institution policy Maintains a safe, secure, and healthy work environment by following standards and procedures; complies with legal regulations Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infections body fluids, chemical disinfectants, radiation and other hazardous substances Assists in the maintenance of medical charts (filing, Op Reports, test results, home care forms) May schedule surgeries/procedures or work in conjunction with Surgical Coordinator verifying times with patients; preparing charts, pre-admissions and consent forms as necessary May be required to perform proficient phlebotomy duties May perform appointment scheduling and registration for patients as well as updating patient demographic and insurance information May maintain, adjust and confirm patient office schedule May check and verify & update insurance information utilizing internal or web sources May perform and verify pre-certification for procedures as necessary Performs other duties as assigned Qualifications High School Diploma/GED with completion of Training Certificate from an approved Medical Assistant certificate course. Certified Medical Assistant preferred. Three to nine months of healthcare experience preferred.
