Summary The nurse is professional registered nurse who is responsible for the coordination of clinical care for patients treated. The nurse facilitates the growth and maintenance of the clinical program in direct collaboration with the Medical Director(s). The lead also is responsible for providing regulatory and quality oversight to the staff as well as participation in division initiatives under the direction of the Director of Nursing. Facilitates care of patients throughout the Rush system utilizing interdepartmental collaboration. This position is a 100% onsite opportunity. You will need to be based in the greater Chicagoland area. Located in Chicago’s Streeterville neighborhood, UChicago Medicine River East is a state-of-the-art outpatient center featuring a wide range of care services. In addition to those listed below, we offer infusion therapy, lab services, and radiology services, including CT, MRI, mammography, X-ray, DEXA scan and ultrasound. A Registered Nurse, UCM Care Network, is responsible for providing professional nursing care for patients including assessment, treatment, care planning, and medical care evaluation by following established standards, protocols and practices. Assists physicians and staff in coordinating patient care. Functions as primary liaison between patients, staff and physicians. This position will be located in a community-based physician practice. Essential Job Functions Obtains patient consent for care and ensures patient confidentiality Obtains and documents health history, information on chief complaint, vital signs, and health risk factors and assesses disease condition Provides patient care based on practice guidelines, standards of care, and federal/state laws and regulations Executes prescribed treatments and medical interventions, administers prescribed medications, and monitors and documents treatment progress and patient response Delegates as allowed under the Nurse Practice Act. Refers to/consults with physicians, other health providers, and community resources to prevent/resolve problems or concerns Educates patients and families about health status, health maintenance, and management of acute and chronic conditions Documents patient assessment and intervention data using established medical record forms/automated systems and documentation practices Participates in multidisciplinary teams to improve patient care processes and outcomes Tracks quality assurance data and monitors for acute and chronic care management Maintains/reviews patient records, charts and other pertinent information Posts tests and examination results Responds to/refers incoming patient treatment-related phone calls Maintains patient confidentiality Performs other duties assigned Required Qualifications Must be an actively licensed Registered Nurse in the state of Illinois Minimum of one year professional nursing experience Completion of nursing training from an accredited school of nursing Active BLS certification from the American Heart Association (AHA) Preferred Qualifications A Bachelor's degree in Nursing Prior clinic experience Position Details Job Type/FTE: Full-Time 1.0 FTE Shift: Days - Monday - Friday, 8s (7:30 am - 4 pm) or 10s (7:30 am - 6 pm) Work Location: Onsite - River East Unit/Department: Cardiology CBA Code: NNU National Nurses United - UCMG River East
