Responsibilities Nurse Practitioner - Fort Myers, FL Horizon Health Corporation is seeking a Nurse Practitioner for an Intensive Outpatient Program and Partial Hospitalization Program in Fort Myers, FL . This unit is a 6 to 15 beds mental health program. Under the general supervision of the Medical Director, the Nurse Practitioner (NP) will provide clinical services to the behavioral health program and is primarily responsible for diagnosing and treating patients; and providing continuing education to patients and their families according to approved protocols. The NP will assess the patients by examining medical history, physical exams and behavioral health evaluations; and ordering or performing diagnostic tests, and within the bounds of scope of practice as delineated by both federal and state regulations/guidelines. Responsibilities: Complete patient diagnoses, treatment, education and referrals in accordance with Hospital policies Perform treatment planning and intervention Medication orders and reconciliation Work with the treatment team in admitting and discharging patients to and from the Program Conduct physical exams of the patient during admission and attend to basic medical needs/issues; work with hospital specialists to manage more complicated medical conditions Perform admission work-ups within twenty-four (24) hours of admissions and provide appropriate documentation as may be defined by Hospital policies Conduct rounds on patients Perform as part of the treatment team in planning, coordinating and implementing nursing care activities for patients Ensure optimal patient care Assist Program leadership in assessing, creating and implementing advanced practice provider procedures, or ensuring existing procedures meet federal and state regulations and guidelines for Nurse Practitioner scope of practice Performs other duties as assigned/required by this position. Dialysis RN: Sets up and operates dialysis machines, monitors patients before, during, and after treatment, and adjusts treatment parameters as needed. Takes vital signs, monitors signs of complications, and responds to changes in patient condition. Educates patients and families about kidney disease, dialysis procedures, and the importance of adhering to treatment plans, diet, and medication. Inspects and maintains dialysis machines and equipment. Cath Lab RN: Pre-Procedure: Reviews medical history, assesses patient's overall health, and prepares them for the procedure. Educates patients and families about the procedure and what to expect. Verifies that surgical consents have been signed. During the Procedure: Assists the Cardiologist during the catheterization process. Closely monitors the patient's vital signs, hemodynamic data, and sedation levels. Ensures proper functioning of equipment and supplies. Manages potential complications and responding to emergencies. Post-Procedure: Continues to monitor the patient's vital signs and overall condition after the procedure. Administers post-procedure medications as needed. Performs other duties as assigned.
