Principal Responsibilities • Works in collaboration with the Interdisciplinary Team to assess the needs of the resident; Provides interdisciplinary schedule for MDS assessments and care plan reviews as required by governing agencies. • Ensures that the Interdisciplinary team makes decisions for either completing or not completing additional MDS, assessments based on clinical criteria as identified in the most recent version of the RAI User’s Manual. • Assist with coordination and management of the daily stand up meeting, to include review of resident care and the setting of the assessment reference date(s). • Complies with federal and state regulations regarding completion and coordination of the RAI process. • Monitors MDS and care plan documentation for all residents; ensures documentation is present in the medical record to support MDS coding. • Maintains current MDS status of assigned residents according to state and federal guidelines. • Maintains the frequent and accurate data entry of resident information into appropriate computerized MDS programs. • Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident and family members. • Attends interdisciplinary team meeting, quality assurance and other meeting in order to gather information, communicate changes, and maintain and update records. • Assists DON or designee with identification of a significant change, physician orders and verbal reports to assure that the MDS and care plan are reflective of those changes. • Prepares scheduling, notice of resident care planning conferences, and assists DON in communication of outcomes/problems to the responsible staff, resident, and/or responsible party. • Continually updating knowledge base related to data entry and computer technology. • Completes electronic submission of required documentation to the state database and other entities per company policy. • Corrects and ensures completion of final MDS and submits resident assessment data to the appropriate State and Federal government agencies. • Assigns, assists, and instructs staff in the RAI Process, PPS Medicare, Medicaid (Case Mix as required) and clinical computer system in relation to these processes. • Maintains confidentiality of necessary information. • Other duties, responsibilities and activities may change or assigned at any time with or without notice. Qualifications • Graduate of an approved Registered Nurse program and licensed in the state of practice, required. • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. • Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required. • Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set. • Knowledge of the care planning process. • Experience with MDS 3.0, preferred. Our Visiting After Hours Nurse (RN) works with the In-Home Services and Primary Care team reporting into the Director of After-Hours Nursing and Comfort Care Services team. After Hours - 4:30pm to 8am Salaried position with mileage reimbursement at $.65/mile Provides phone triage and drives to participant’s home/community for direct care as needed. Will support enrolled participants for their designated PACE center From incoming calls supports on-call assessments with participant/family initiating appropriate intervention, including identifying and respond to emergency situations with the ability to handle on-call assessments and triage appropriately. Job Description The Caregiver meet community residents’ needs by helping with activities of daily living. Responsible for the delivery of quality care by encouraging independence and ensuring each resident’s physical, social, cultural, and intellectual well-being. Location: Balfour Riverfront Park | 1500 Little Raven St, Denver, CO 80202 Schedule: - Full-Time AM | 6 AM to 2:15 PM | Friday to Monday or Thurs/Fri/Sun/Mon - Full-Time PM | 2 PM to 10:15 PM | Saturday to Tuesday Pay: $19.29-$20/hour (depending on relevant years of experience) Hiring Bonus: $500 What will I do every day? Monitor residents’ well-being in a general way and report any changes in condition to the appropriate associate/department immediately Assist residents with simple requests as needed or find appropriate associate to assist Learn resident preferences for opportunities to provide exemplary customer service, exceed resident expectations and enhance resident satisfaction Supervise or assist with bathing, dressing, grooming, meals, ambulation and other activities of daily living Provide companionship and develop strong resident relations Respond to emergencies according to policies and procedures Help residents with assistive devices (walkers, wheelchairs, hearing aids, etc.) Encourage self-care of residents Document care provided to residents consistent with documentation standards Provide input to the Care Services Director and/or the Resident Care Coordinator/Dementia Care Coordinator to assure that resident individual service plans are current Review24 Hour Report and care check lists regularly Maintain resident confidentiality and privacy Practice standard precautions, infection control, and safe work practices Observe and monitor residents to provide or assist with documentation regarding resident status Cover the Customer Service desk in the Care Services lobby, if necessary Answer telephone according to company policy and standards Support, lead and participate in resident-centered wellness programs Resolve resident questions according to set policies and procedures Greet and direct family members, potential residents, prospects and other individuals to the appropriate staff contacts in the community Assist and participate in community special events and marketing events Assist residents in meeting their social needs through interaction with staff, family and other residents including encouraging and reminding residents of wellness programming Assist in the general upkeep of the community common areas Maintain upkeep of residents’ rooms (e.g., making bed, taking out trash, washing dishes) Assist with resident laundry as assigned Support open and honest communication with and between all associates Work as part of the team within your department and across the community Maintain high standards of personal appearance and grooming, which includes wearing the proper uniform and name tag when working Look for ways to reduce costs and create efficiencies on a daily basis Ensure regulatory compliance and report any issues or concerns immediately to supervisor Support continuous improvement Adhere to all regulations, laws and company standards, including the Associate Handbook Understand and support the community sales process Perform other related duties as required What will I need to be successful in this role?
