The Licensed Practical Nurse (LPN) provides direct patient care in accordance with applicable laws and organizational policies. The LPN supports the delivery of compassionate, high-quality care in hospice and palliative care settings, performing assigned clinical tasks and collaborating with interdisciplinary teams to ensure optimal outcomes for patients and caregivers. Position Details Days/Hours : Nights; 3x12 shifts, 8pm-8am weekdays with some weekends, usually one weekend a month Work Coverage Area: Lake City Area and Surrounding Counties; Home Care with some local Assisted Living Facilities Certification Required : ** IV Certification ** Compensation : $27.00 - $32.00/hr, plus extra paid driving mileage; full benefits package plus 23 days annual PTO Sign On Bonus Eligible : Yes $2,000 Primary Job Responsibilities Completes assessment of patients regularly and on-going as patients’ conditions warrants. Provides patient care in accordance with physician orders and patient/family goals, directed by the Care Plan; provides post-mortem care per policy. Demonstrates awareness and maintenance of Universal Precautions. Provides emotional support to patient/caregiver. Teaches patient/caregiver as appropriate to situation, plan of care. Demonstrates knowledge and appropriate use of medications, including medication administration. Evaluates services provided to patients, apprising Inpatient Unit Manager /designee if compliments/concerns or opportunities for improvement are expressed. Facilitates transfers into the Inpatient Facility (if appropriate) further ensuring continuity of care of Hospice services. Completes documentation so that: Charts patient condition/changes every 1 hour during shift. (For CIC cases only). Documentation completed is reflective of patient care given, services provided, relevant to Continuous Care criteria and death/discharge process. Any concerns noted, patient status; is legible, neat; is accurate, thorough, completed in a timely manner. Documents relevant to the Medication Administration, Narcotic Administration, and accountability and destruction of medication following patient death. Care Plans are kept current, applicable to patient and patient needs. Incident Reports are completed accurately and submitted immediately to the Nursing Support Service Manager/designee. Licenses & Certifications IV Certification required Valid LPN License State of Florida driver’s license with driving record acceptable to Community Hospice insurance carrier. Current BLS Education & Experiences Any combination of education and/or experience that would provide the required skill and knowledge for successful performance would be acceptable. Typical qualifications would be equivalent to: Graduate from an accredited Practical Nursing Program Minimum of one (1) year experience in acute care setting, long term care setting or home health setting as an LPN.
