Specialty Essential Functions Level of Care Management Reviews clinical criteria of all ED / Direct Admission patients for appropriate patient status (i.e. inpatient, observation services, outpatient testing/treatment) based on medical necessity criteria. Discusses recommendations for level of care with ED physician and attending physician. Discusses options for care with patients, families, and physicians involved in their care. Remotely monitors bed board (where applicable). Psychosocial Assists patients and their families through the complex emergency room environment and/or the hospitalization process. Provides emotional support to patients and their families/support system as needed. Connects patients and their families/support system to community services as needed and appropriate. Discharge Planning / Case Management Plans, coordinates, and implements discharge plans for ED and ED hold patients, including but not limited to, referrals to skilled nursing facilities, personal care homes, home health, DME, drug and alcohol treatment facilities, transportation, or community-based services. Coordinates authorization for admitted patients and/or patients transferred from the ED. Provides case management follow through for the Unit Based Case Manager on cases requiring follow-through after normal business hours. Professional Development Attend meetings as appropriate and meet regularly with supervisor to exchange pertinent information. Participates in continuing education activities, remaining knowledgeable in area(s) or expertise. Maintains a high level of ethical conduct regarding confidentiality, dual-relationships and professional stature. Recognizes and progresses the plan of care when an alternative level of care is appropriate. Collaborates with the attending physician and multidisciplinary healthcare team to facilitate the progression of the plan of care. Cognizant of payor requirements for all patients in assigned caseload. Responsible for understanding and communicating plan benefit limits/availability to patient or their representative in management of case as necessary. Responsible for accurate and timely documentation in recognized databases to support Clinical Resource Management and ED components for each patient in assigned caseload. Denial Management Initiates contact with the attending physician on cases that do not meet InterQual Level of Care SI/IS indicators in order to clarify plan of care. Educates physicians and other members of the healthcare team on the application of InterQual Level of Care Criteria to support an acute level of care and provides alternatives to acute care as appropriate. Notifies the unit-based case manager of all real or potential cases in which an adverse determination or complex psychosocial situation is anticipated. Identifies and facilitates transition to the most appropriate alternate level of care. Required Qualifications Graduated of an accredited nursing program. Two (2) years recent healthcare/clinical experience, or recent clinical experience in an emergency department, case management, or utilization review position. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. The Team Lead assists with surgical procedures in the Operating Room as a scrub member of the surgical team under the direction of a registered nurse and a surgeon. Essential Job Functions Collaborates with Surgeon, Clinical Specialty Coordinator, Supervisor, and Director to develop service line goals including: Doc preference cards, efficiency metrics, orientation assistance, and overall service line excellence Delivers direct and indirect patient care in accordance with hospital/departmental policies, procedures, protocols and/or their professional standards of care. (b) Additional Requirement - An ST employed or contracted with the facility and not certified under section 501(1)(i) or has let the individual's certification lapse shall: (1) complete 30 hours of relevant continuing education every two years in order to remain eligible to practice surgical technology; and (2) be responsible for proving, to the satisfaction of the health care facility, that the individual complies with this subsection. (c) Verification-- A facility that employs or contracts with an ST shall verify the ST meets the continuing education requirements. Section 503. Exception. A facility may employ or contract an ST who does not meet the requirements of section 501 to practice surgical technology if: (1) after a diligent and thorough effort has been made, the facility is unable to employ or contract with a sufficient number of qualified STs who meet the requirements of section 501; and (2) the health care facility makes a written record of its efforts under (1) and retains the record at the facility.
