MDS Coordinator - Nursing Home Located in Antioch, TN Salary Up To $115K Per Year; * Based on experience * Responsibilities : Conduct and coordinate the development and completion of the resident assessment (MDS) Maintain and periodically update written policies and procedures that implement MDS and care plan. Assist the resident in completing the care plan portion of the resident’s discharge plan. Develop and implement procedures with the Director of Nursing Services to inform all assessment team members of the arrival of newly admitted residents. Assist Facility directors and supervisors in scheduling the resident assessment and care plan meetings. Assist in determining appropriate treatment, selecting activities and exercises based on medical and social history of residents. Participate in the development and implementation of resident assessments (MDS) and care plans, including quarterly and annual reviews. The specialist will respond to behavioral events throughout their shift, working with patients who exhibit behavioral challenges, have mental health diagnoses, or experience difficulties adhering to medical care. When not actively responding to crises, the specialist will collaborate with the team to identify patients at risk for behavioral events and provide proactive support to patients, families, and staff to prevent crises. Depending on the assignment, responsibilities may include: Providing medication reminders Assisting with approved blood glucose monitoring Documenting readings when required Observing food and fluid intake Monitoring the client’s physical, emotional and behavioral condition Reporting pain, swelling, bruising, redness or skin concerns Reporting falls, injuries or changes in mobility Reporting confusion or unusual behavior Following established emergency procedures Contacting the Helping Hearts office immediately when concerns arise Completing accurate electronic documentation Caregivers must report concerns rather than independently diagnose or treat them. Caregivers must not assume that someone else has already reported a change in condition. Medication Reminders Medication support is limited to the responsibilities authorized in the client’s approved non-medical care plan. Caregivers may: Provide scheduled reminders. Assist with an approved prefilled medication planner. Place approved medications in a cup when directed. Remind the client to self-administer their medication. Document reminders and client responses. Report refusals, missed doses or other concerns. Caregivers must not independently administer medication, change medication instructions or provide medical advice outside their authorized scope. Transportation When required and authorized, caregivers may: Transport clients to appointments. Assist with approved errands. Transport clients to social activities. Help clients enter and exit the vehicle. Secure mobility equipment appropriately. Follow all client-specific transportation instructions. Maintain a clean, safe and insured vehicle. Report any transportation concern immediately. Caregivers may only transport clients when the assignment has been approved and all transportation requirements have been satisfied. Night and Overnight Shift Expectations Caregivers working night or overnight assignments must remain fully awake, alert and attentive throughout the entire shift.
