Available daytime shifts range from 4 to 12 hours. Shift lengths, start times, end times and available days vary according to each client’s care plan, personal preferences and scheduling needs. Applicants must provide their complete daytime availability so they can be considered for appropriate assignments. Once a caregiver accepts a permanent or temporary assignment, they must be able to work the complete scheduled shift consistently. Important Pre-Interview Document Requirement Applicants must possess and present physical copies of all required employment documents during the pre-interview document-verification process. Applicants who do not have the required physical documents available will not be able to move forward in the interview and hiring process. Assignments are matched according to the caregiver’s experience, skills, availability, location and ability to safely complete the care plan. Companionship and Household Support Some clients may primarily need: Meaningful conversation and companionship Social engagement and recreational activities Meal preparation and cleanup Grocery organization and errands Light housekeeping Laundry and linen changes Organization of the client’s immediate living space Medication reminders Transportation to approved appointments and activities Safety observation and general supervision Personal Care and Activities of Daily Living Other clients may require hands-on assistance with: Bathing or showering Sponge baths and personal hygiene Dressing and undressing Grooming, hair care and oral care Toileting and incontinence care Changing protective undergarments Meal assistance and hydration reminders Walking and mobility assistance Wheelchair support Entering and exiting vehicles Repositioning and comfort measures Maintaining dignity, privacy and independence during personal care Dementia and Memory-Care Support Some assignments may involve clients living with dementia, Alzheimer’s disease or other cognitive conditions. Responsibilities may include: Providing calm and consistent supervision Offering appropriate redirection and reassurance Supporting established routines Monitoring for confusion, agitation, wandering or changes in behavior Encouraging safe and appropriate activities Maintaining a quiet, respectful and supportive environment Following client-specific communication and safety strategies Immediately reporting significant behavioral or cognitive changes Caregivers assigned to dementia or memory-care clients must remain patient, attentive and aware of the client’s location and safety throughout the entire shift. Mobility, Transfer and Higher-Assistance Support Some clients may have limited mobility, be at high risk for falls or require significant physical assistance. Depending on the care plan and caregiver training, responsibilities may include: Assisting with walking and balance Supporting wheelchair mobility Assisting with bed mobility and repositioning Helping clients move between the bed, wheelchair, chair, commode or toilet Using an approved gait belt when directed Assisting with sit-to-stand or stand-to-sit transfers Using a mechanical or Hoyer lift only after required training and authorization Following all one-person or two-person transfer requirements Requesting assistance when a transfer cannot be completed safely Caregivers must never attempt an unsafe transfer or use equipment they have not been trained and authorized to operate. Non-Medical Health Support Depending on the assignment, caregivers may also be responsible for: Providing medication reminders according to the approved care plan Assisting clients with approved blood-glucose monitoring Documenting readings when required Observing food and fluid intake Monitoring for changes in physical, emotional or behavioral condition Reporting falls, injuries, pain, swelling, bruising, redness, skin concerns, confusion or unusual behavior Following established emergency and office-notification procedures Accurately documenting all completed tasks, observations and concerns Helping Hearts provides non-medical personal support. Caregivers must remain within their authorized role and follow the client’s approved care plan. Caregivers may be considered for additional assignments when their availability, experience and location match current needs. Once a caregiver accepts an assignment, dependable attendance and completion of the entire scheduled shift are required. Employment Qualifications Applicants must: Be at least 18 years old. Possess a high school diploma, GED or equivalent. Have professional caregiving, personal-care, home-care or related experience. Demonstrate compassion, patience, professionalism and dependability. Follow written care plans and office instructions. Communicate professionally with clients, office staff and approved facility personnel. Complete accurate electronic documentation during every shift. Have reliable access to a smartphone. Maintain client confidentiality. Remain attentive and awake throughout every assigned shift. Follow all attendance, call-off, clock-in, clock-out and documentation requirements. Safely perform the physical responsibilities of the accepted assignment. Complete required orientation, training and assignment-specific instruction. CPR and First Aid certification are not listed as pre-interview document requirements for this position. Experienced Caregiver Needed for Franklin-Area Client Location: Franklin, Tennessee Schedule: Every Monday, Wednesday, and Friday Hours: 12:00 PM–5:30 PM Weekly Hours: Approximately 16.5 hours Position Type: Consistent, part-time home-care assignment Helping Hearts is seeking a reliable, compassionate, experienced, and highly attentive caregiver to support a client in the Franklin area. This client requires full assistance with activities of daily living, including hands-on transfer assistance, and is considered a significant fall risk. We need a caregiver who understands that fall prevention requires constant awareness, close supervision, careful assistance, and proactive safety measures throughout the entire shift. The selected caregiver must be available to work all three scheduled days Monday, Wednesday, and Friday from 12:00 PM to 5:30 PM. Consistent attendance is essential because the client depends on a stable routine and familiar caregivers. The client also has a small dog in the home. Applicants must be comfortable entering and working in a home with a small household pet. At Helping Hearts, caregiving is more than a job, it is a calling. We are looking for someone who will treat the client with dignity, patience, compassion, and respect while remaining actively engaged and focused on the client’s safety. Primary Responsibilities Full Assistance with Activities of Daily Living The caregiver will provide respectful, hands-on assistance with the client’s daily personal-care needs as outlined in the client’s care plan. Responsibilities may include: Bathing or personal-hygiene assistance Dressing and undressing Grooming and oral-care support Toileting and incontinence care Mobility assistance Positioning and repositioning Safe transfer assistance Wheelchair and mobility-device assistance Meal preparation and hydration reminders General comfort and personal-care support Maintaining the client’s privacy and dignity during all care tasks Care must be provided patiently and professionally without rushing the client. The caregiver must follow the established care plan and use only approved transfer techniques and equipment for which the caregiver has received proper training. Safe Transfer Ability Is Required This client requires hands-on assistance with transfers. Applicants must have the physical ability and willingness to safely assist the client with transfers, including: Wheelchair to commode and commode to wheelchair Wheelchair to bed and bed to wheelchair Wheelchair to chair or other approved seating Sit-to-stand and stand-to-sit transfers Toileting-related mobility and clothing-management assistance Repositioning for safety, hygiene, and comfort Transfers using approved equipment when required by the care plan Previous experience using a Hoyer lift, sit-to-stand lift, or other mechanical transfer equipment is preferred but not required. Applicants without this experience must be willing and able to complete all required hands-on training and demonstrate safe use of the equipment before independently performing mechanical-lift transfers. During every transfer, the caregiver must: Explain the transfer process to the client Make sure the client is positioned correctly Properly position the wheelchair or commode Lock the wheelchair brakes before beginning Move footrests, cords, furniture, and other obstacles out of the way Clear and inspect the transfer area Use proper body mechanics Use approved transfer equipment when required Closely monitor the client’s balance and comfort Move slowly and avoid rushing or pulling the client Stop immediately if the transfer becomes unsafe Transfers must never be rushed or attempted in an unsafe manner. The caregiver must follow the client’s care plan and use any required gait belt, Hoyer lift, sit-to-stand lift, or other approved transfer device only after receiving proper training and authorization. If a transfer requires two caregivers or cannot be completed safely, the caregiver must not attempt it alone. The caregiver must keep the client safe and contact the Helping Hearts office immediately for assistance and further direction. Applicants must be able to perform the essential transfer responsibilities of this position safely and consistently, with or without reasonable accommodation. Proactive Fall Prevention and Safety Monitoring Because this client is at significant risk for falls, the caregiver must remain alert, attentive, engaged, and safety-focused throughout the entire shift. This is not a passive companion assignment or a position where the caregiver can sit nearby without actively observing and assisting the client. The caregiver will be expected to: Remain aware of the client’s location, mobility, and immediate needs Provide close supervision during standing, walking, toileting, and transfers Stay within a safe assisting distance whenever the client is moving Encourage the client to request assistance before attempting to stand or transfer Maintain a clear and hazard-free environment Check for spills, cords, loose rugs, clutter, poor lighting, and unsafe furniture placement Make sure mobility devices are properly positioned and within reach Make sure the client has appropriate footwear during mobility Keep commonly used items safely within the client’s reach Remain especially attentive when the client is tired, weak, distracted, or attempting to toilet Never leave the client unattended during a task when supervision is required Avoid personal-phone use or other distractions that interfere with active supervision Immediately report changes in mobility, balance, strength, or transfer ability Immediately report every fall, near fall, injury, or safety concern Complete all required incident documentation accurately and promptly The caregiver must never attempt an unsafe lift or transfer. If the client cannot be moved safely according to the care plan, the caregiver must protect the client from further injury and contact the Helping Hearts office immediately for direction. Active Engagement and Companionship The client needs a caregiver who will be present, attentive, conversational, and genuinely engaged. Meaningful companionship is an important part of this assignment. The caregiver should: Initiate friendly and appropriate conversations Offer reassurance, encouragement, and emotional support Participate in activities the client enjoys Help maintain a calm, positive, and familiar daily routine Recognize when the client may be lonely, anxious, uncomfortable, or withdrawn Encourage safe participation in daily activities whenever appropriate Respect the client’s choices, preferences, personal space, and independence Remain patient if the client needs additional time, repetition, or reassurance Provide companionship without becoming distracted by a personal phone, television, or unrelated activities We are looking for someone who understands that quality caregiving includes both physical assistance and meaningful human connection. Meal Preparation, Snacks, and Hydration Depending on the client’s care plan and preferences, responsibilities may include: Preparing simple meals and snacks Encouraging appropriate food and fluid intake Providing hydration reminders Assisting with meals when required Cleaning food-preparation areas after use Washing dishes and returning items to their proper locations Observing and reporting appetite changes, swallowing concerns, nausea, or refusal of food or fluids Following all dietary instructions listed in the client’s care plan Attention to cleanliness, nutrition, hydration, and the client’s comfort is essential. Concerns must be reported immediately. Caregivers must not wait until the end of the shift to report an urgent change, fall, near fall, injury, or safety concern. Small Dog in the Home The client has a small dog. The rapid response team (also known as the Critical Care Outreach Team - CCOT) provides 24 hour coverage across the hospital for episodic care when patients have an acute change in clinical status outside of an ICU. The rapid response/CCOT NP or PA works with the primary team and supervising intensivists to evaluate a patient’s clinical status and identify interventions to stabilize any acute changes while considering moving the patient to a higher level of care. RRT/CCOT is responsible for documentation and data collection related to rapid response and STAT activations as well as education to patients, nurses, and physicians in relation to rapid response and STAT calls ICU Support Team The Critical Care APP works in collaboration with a multidisciplinary health care team. The NP/PA provides healthcare in critical care settings, including surgical and medical ICUs. The APP has didactic education, clinical competency and national certification in identified area of clinical practice. Upon moving to a new ICU, the APP will receive adequate training and support before moving to a more independent role within the ICU for 8 continuous weeks. Each year, the APP will spend blocks of time working on the RRT/CCOT as well as supporting a new ICU for several months where staffing shortages are occurring.
