The Registered Nurse provides professional nursing care to patients within a assigned unit with a commitment to quality and safety. Professional nursing practices, requiring specialized knowledge judgment and skill are performed using the process in accordance with hospital policy, procedures and nursing philosophy. Responsibilities: Provide direct, professional nursing care to specified population of patients in the acute care setting. Develops a patient centered plan of care based on individualized needs. Maintains confidentiality of the patient medical record. Counsel and educate patients and their care partner on treatment plans. Administer medication following the five rights of medication administration. Provide wound care and dressing changes, as applicable. Read and interpret patient information, making treatment decisions, when appropriate. Consult with physicians to decide on best treatment decisions when appropriate Performs nursing interventions to achieve optimal patient outcomes. Observe and record patient behavior and response to nursing interventions. Demonstrates quality, safe and effective patient care. Responsibilities: The LPN must read and interpret documents such as doctors’ orders, medication names, and lab tests. Perform focal assessments for established inpatients. Provide general basic nursing care, such as positioning patients, lifting and turning, assisting in use of bedpan, commode, urinal or bathroom, and weighing. Provide exceptional customer service that aligns with the Planetree philosophy. Perform routine, uncomplicated treatment procedures including enemas, application of hot and cold compresses, dressing changes, wound cleansing, obtaining vital signs including orthostatic vital signs and rectal temperatures and splinting of extremities. Collects and records intake and output. Prepare and administer oral medications as ordered by the physician or APC. Review medication history with patient. Assist with minor medical/surgical procedures in the practice setting. Performance of venipuncture for the purpose of blood and to monitor the patient’s condition before, during and after the withdrawal of blood. Transport patients using wheelchair or stretcher within offices and to/from vehicles. Assist those patients requiring help with ambulation. Identify the need for patient education and advise patients on their treatment plan. Demonstrate the ability to recognize abnormal lab results. Document treatment records for each patient. Immediately report any observed changes in condition or behavior of patients and documents any care given in an acceptable medical legal fashion. Immediately reports any unusual incidents.
