Temporary Offsite Medical Assistant I Ambulatory-Obstetrics And Gynecology Administration-Full-time Day

Description The Medical Assistant I provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. This individual is also responsible for assisting with clinical, administrative and organizational tasks associated with patient care coordination. Responsibilities 1. Assists the provider during patient visit via obtaining vital signs (height, weight, blood pressure, pain scale etc) and documenting vital sign results and transcribing medication information into the electronic medical record (EMR). 2. Obtains prior authorizations for prescriptions and durable medical equipment 3. Drafts prescription requests in EMR and sends to provider for review, completion and processing 4. Performs Phlebotomy services inclusive of collecting and processing blood, urine and stool specimens and use of a centrifuge for spinning bloods 5. Performs POCT (w/the exception of glucose) and documents results in EMR for provider to review 6. Performs daily Quality Control for all POCT equipment 7. Orders clinical supplies when necessary 8. Maintains and stocks exam and consult rooms with clinical supplies and clerical forms used by Providers 9. Assists providers with procedures by preparing supplies and equipment and acting as a chaperone as needed. 10. Perform EKGs when ordered by provider 11. Performs pre-visit planning for Diabetic and CHF patients as per practice guidelines 12. Monitor and maintains refrigerator temperature logs and biomed expirations for medical equipment 13. Participates in daily huddles with MDs and other clinical team members to review daily care management patient lists and discuss patient's treatment plan & management 14. Performs age-specific screenings for Activities of Daily Living, Falls, Alcohol, Tobacco, and/or Depression screenings annually for every patient 15. Enters data into the electronic medical record related to medication adherence, diabetes control, foot/eye exam compliance for provider review and reconciliation 16. Assists providers with distributing educational materials to patients 17. Assists Population Health Management team by verifying patient has completed their colorectal screening, breast screening, cervical cancer screening and other pertinent preventive screens. 18. Retrieves information regarding hospital admission and Emergency Room visits from outside facilities for evaluation by provider for transition of care coordination 19. Assists providers with follow-up related to patients on Coumadin therapy 20. Assists at the front desk as needed: answer telephones, register patients, discharge patients, schedule appointments, copy charts, schedule ancillary tests, and follow up on overdue diagnostic test results. 21. Follows Primary Care Medical Home and MACRA protocols for designated role 22. Performs all other job related functions as assigned. Job Summary Responsible for implementing and supervising nursing services in the medical detoxification and gives direction to unit assistants on shift. Assists in training and evaluation of medical staff. Implements nursing care in a medically supervised facility treating up to 16 individuals going through withdrawal from alcohol, opiates, benzodiazepines, and other drugs, in accordance with program procedures of the organization. Essential Functions Implements nursing care in accordance with program procedures established by the Medical Director and Nursing Supervisor and approved by the Program Administrator. Administer drugs, medications, treatments, inoculations, and tests as allowed by law. Implement verbal physician orders and authenticating in writing, within a reasonable time, and included in the patient medical record. Communicate with physicians, nurse practitioners, nurses, and pharmacists regarding patient care. Pre-screening of new patients for appropriateness for admission, including TB screening. Obtain physical assessment data and health history. Record and document into health record all observations of patient response, following CIW-Ar, COWS or other assessment tools utilized in program protocols Inventorying and caring for patients’ personal possession. Executes prescribed treatments and medical interventions, including transcribing and administering routine and prescribed medications in accordance with procedures established by the Medical Director and Supervising Nurse. Records patient observations and activities in patient records accurately and timely in accordance with nursing standards and organizational policy and procedure. May be designated Charge Nurse for a shift, with oversight of LPNs or nursing assistants. Screens and provides first response to pregnant in detoxification, including obtaining health history on mother, screening for TB and contagious disease. Actively and compassionately interacts with patients to encourage engagement in treatment after completion of detoxification. Provides patient education on health care, as assigned or appropriate. Nurse Practitioner Physician Assistant Job Description Position Summary: Conducts comprehensive health assessments, demonstrating a high level of autonomy and expert skill in the diagnosis and treatment of complex responses of individual, families, and communities to actual or potential health problems. Formulates clinical decisions to manage acute and chronic illnesses and promote wellness. Integrates education, research and management, leadership and consultation into clinical role and functions in collegial relationships with nursing peers, physicians, professionals and others who influence the health care environment. Provides health care in accordance with standards of practice as delineated by state practice guidelines and national board certification organizations. Major Responsibilities: Provides diagnostic and assessment services. Secures a health history from patient and/or family, records findings, and evaluates them. Performs a complete in–depth physical examination. Orders/performs pertinent diagnostic tests based on age and history. Analyzes data collected to determine health status and identify differential diagnoses based on history, physical exam, and clinical findings. Partners with physicians regarding proposed plan of care. Formulates this plan of care with the patient. Manages therapeutic regime as outlined by established protocols for patients with acute illnesses. (Protocols are guidelines concerning patient care that are established between the physician and nurse practitioner. The guidelines outline subjective and objective findings, treatment plan and follow–up). Reassesses and modifies plan as necessary to achieve medical and health goals. Confers with physician for clinical direction as outlined by protocols. Documents patient care outcomes to determine effectiveness of plan of care. Communicates outcomes with physician. Position Qualifications: License/Certification/Education: Required: A Bachelor’s degree and Graduation from an accredited Physician Assistant program. Current Massachusetts license, issued by the Physician Assistant Board, and current NCCPA certification. A Master’s degree and graduation from an accredited Nurse Practitioner program. Preferably ACNP/AGACNP Acute Care/Critical Care clinical experience required Board Certification by the American Nurses Credentialing Center (ANCC) or the American Association of Nurse Practitioners (AANP).
