Reviews all medical data prior to delivering patient care, documents and communicates patient status and assesses needs to the Home Care Nurses. Under direction of Home Care RN, participates in the development of a patient care plan which will facilitate health care, promote patient comfort and well-being, and includes patient/ family. Communicates appropriate information to patients, peers, managers, physicians, department, and other agencies involved. Integrates problem solving methodology and quality improvement in approaching problem areas related to patient care and organizational processes.
