Job Description The CHHA Department is currently seeking a full time CHHA Registered Nurse to join their team! Eligible newly hired UFT nurses will receive a $6,000 hiring bonus, distributed in two phases: the first installment of $3,000 will be provided after successfully completing six months of employment, followed by a second $3,000 payment upon reaching one year of service. Job Summary: Responsible for providing and coordinating multidisciplinary health services for patients in the community. Works with the physician and other team members in implementation of plan of care. Duties And Responsibilities Provides and coordinates cost-efficient patient care that is and reimbursable under the patients’ insurance plan and approved by a physician. Verifies insurance plan information, crosschecks patient Insurance ID card at home. Accepts case assignment depending on clients’ needs regardless of geographic location. Case assignment is all inclusive of CHHA (Certified Home Health Agency) requirements, including initial assessments, revisits and documentation on each day of service. Travels to patients’ homes and/or other facilities using approved transportation to deliver direct care to patients Admits patients in the program as needed and in a timely manner. Ensures in-person completion of the Oasis that accurately reflects clinical and functional status of the patient. Uses diagnosis coding skills to accurately rate the acuity of each Evaluates patient diagnosis and works with Director, Patient Services and physician to assign appropriate primary diagnosis. Participates in bi-weekly and PRN meetings to report adherence to frequency and duration of services as per physicians’ order. Reports patients’ progress to team in order for discharge date to be anticipated and set. Communicates frequently with PT/OT to ensure adherence to therapy at the frequency and duration ordered by the physician. Acts as a coordinator of the multidisciplinary team providing care to patients. Establishes a plan of care with input from appropriate multidisciplinary health team members based on goals mutually acceptable to the patient and family. Documents content and process of all services provided. Monitors the effect of the care plans on the patient health status and informs physician and other personnel of changes in the clients’ needs. Appraises care provided, revises goals and interventions to patients’ care as needed. Educates the patient and family regarding the disease process, self-care techniques and prevention strategies. Initiates and maintains written and verbal communications to assure optimal patient care. Provides supervision of workers providing health and personal care services through visits to patients’ home. Maintains updated Home Health Aide (HHA) plan of care and reviews same with HHA and patient/family. Communicates with HHA on a regular basis regarding interdisciplinary plan of care, patients’ goals and progress. Documents initial and bi-weekly supervision of HHA services. Plans, facilitates and documents patients’ discharge from the program. Manages caseload as assigned in Manhattan. Skills & Requirements Specialized Skills: Demonstrates competency in the provision of nursing care. Demonstrates knowledge of the PPS Medicare system. Demonstrates knowledge of the Oasis and impact on payment. Demonstrates flexibility and the ability to prioritize responsibilities. Demonstrate ability to document and coordinate patient care. Job Details Neuro ICU Physician Assistant and Nurse Practitioner Good Samaritan University Hospital – West Islip, NY (Nurse Practitioner or Physician Assistant) Per Diem Position Summary The Neuro ICU Advanced Practice Provider (APP) delivers comprehensive, high-acuity care to patients with complex neurological and neurosurgical conditions at Good Samaritan University Hospital, an academic tertiary-care center within Catholic Health. The APP works collaboratively with intensivists, neurosurgeons, neurologists, and critical care RNs to manage critically ill patients, perform advanced procedures, and optimize outcomes in a fast-paced Neuro ICU. Key Responsibilities Clinical Care & Patient Management Perform comprehensive neurologic and critical care assessments on Neuro ICU patients, including acute stroke, traumatic brain injury, subarachnoid hemorrhage, status epilepticus, brain tumors, spinal cord injury, and complex post-operative neurosurgical patients. Formulate and implement evidence-based medical management plans in collaboration with attending physicians. Interpret diagnostic studies including CT, MRI, EEG, continuous neuromonitoring, and lab results. Manage advanced neuromonitoring devices such as: Intracranial pressure (ICP) monitors External ventricular drains (EVDs) Licox and cerebral perfusion monitoring systems Initiate and titrate critical care infusions (vasoactive, sedation, hyperosmolar therapies, anticonvulsants). Respond to emergencies including code strokes, rapid response activations, seizures, airway compromise, and acute neurologic deterioration. Procedures (Depending on training & privileges) Central venous catheter placement Arterial line insertion Lumbar puncture Ventilator management and airway stabilization Bedside ultrasound for procedural guidance Removal of EVDs or drains per neurosurgical protocols (if credentialed) Communication & Collaboration Participate in daily multidisciplinary rounds with neurosurgery, neurology, neurointensivists, nursing, respiratory therapy, rehab services, pharmacy, and case management. Communicate clinical plans clearly to patients and families; provide education and support regarding neurological diagnoses. Serve as a clinical resource to nursing staff and contribute to ongoing improvement of Neuro ICU protocols. Description The Transplant Coordinator is a Registered Professional Nurse with specific knowledge and skills who participates in the coordination of clinical programs and services for designated patient populations within the Transplantation Institute. Responsibilities Responsible For Development, implementation, coordination and evaluation of clinical programs and services for designated patient populations. Principal Duties and Responsibilities Clinical Practice 1. Coordinates the multidisciplinary screening and evaluation of patients seen in the transplant practice at MSH and approved satellites. The multidisciplinary team includes the transplant surgeons, hepatologists, nephrologists, social work, nutrition, pharmacy and finance. Coordinators work with their assistants to facilitate other medical specialty visits as needed. 2. Ensures all necessary lab tests are performed on potential transplant candidates and that results are updated as required (including bloodwork sent to Rogosin for tissue typing). 3. Participates in making recommendations regarding placing as patient on the transplant waiting list. Educates patients on the benefits of living donation (when applicable). 4. Communicates with all active transplant candidates regarding waiting list status on an ongoing basis. Ensures all medical clearances are updated regularly. 5. On call responsibilities include facilitating all organ offers from UNOScommunicating with surgeons, fellows and patients to ensure timeliness of all organ offers. Coordinators also answer patient calls after hours, on holidays and weekends. 6. Description 3037197: Staff Nurse - Interventional Radiology - Mount Sinai Hospital - Full Time Night The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. 3. Integrates relevant assessment and intervention skills in the delivery of nursing care. 4. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. 5. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care and delegating to others as appropriate. 6. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Administrator. 4. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. 6. Facilitates patient throughput on a shift basis.
