Description The Assistant Nurse Manager is a Registered Professional Nurse with broad knowledge and skills who acts as clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Clinical Nurse Manager. Responsibilities Patient Care Leading nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. Ensures the integration of PHR into clinical activities to enhance Relationship Centered Care. Validates staff members round hourly on all patients by direct observation and patient feedback. Assesses staff competency in PHR and provides consistent meaningful feedback with more focused feedback as needed. Conducts rounding of all patients on a daily basis (Monday-Friday.) Communicates quantitative and qualitative information regarding the patient experience and staff engagement to the staff and the Interdisciplinary Patient Care Team that leads to the development and implementation of an action plan. Collaborates with the physician dyad and escalates concerns as appropriate. Follows up on all patient concerns in a timely manner and provides service recovery as needed. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. Patient Experience Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions but not limited to: Admission Welcome, Bedside Shift Report, HELP, AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding (PHR), Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. Huddles with staff at the start of each shift, identifying high-risk patients who require special attention. Communicates to nursing leadership concerns or issues identified during huddle and throughout shift. Initiates service recovery for patients and family members who have concerns and escalates to Clinical Nurse Manager/Associate Director of Nursing/designee or Patient Relations as needed. Interprets and communicates patient experience scores and benchmarks and keeps staff informed of progress and areas of opportunity. Considers the patients values, preferences, expressed needs and knowledge in all aspects of care. Empowers staff to use evidence-based practices to increase understanding of patient's perceptions of care. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. Quality and Safety Designs, supports and leads a safe, therapeutic and efficient patient-centered care environment for professional practice. Identifies and investigates issues or variances in practice/operations by participating in the RCA process and implements corrective action plans. Fosters a fair and open culture that encourages error and near-miss occurrence reporting. Leads interdisciplinary quality improvement teams using the principles of high reliability. Reviews unit progress, changes and compliance with quality and safety metrics with clinical nurses and other members of the healthcare team. Responds to escalations of potential safety hazards or gaps from best practice by correcting concerns following institutional chain of command protocol. Monitors institutional information technology infrastructure and corrects gaps in clinical nurse documentation. Leads staff participation in quality and safety initiatives and mandated compliance measures. Demonstrates accountability for all unit quality and safety practices and compliance, and ensures required evaluation of staff competencies is maintained. Operations and administration Supports implementation of the mission, vision, goals and objectives of the department, throughout the clinical service and individual patient care units. Provides administrative and clinical oversight to ensure that all activity (staffing, support services, inter disciplinary collaboration, resources and financial viability) supports the goals of the organization. Works collaboratively with physician dyads and the healthcare team to promote patient care and regulatory standards. Maintains a nursing unit in compliance with all regulatory agency mandates, including the regulations of NYS DOH and The Joint Commission. Oversees 24 hour patient care delivery, throughput, staffing and processes to ensure efficient and effective operations. Prepares, implements and monitors unit budgets, including capital equipment allocation, budget variances, management of OT and vacancies according to MSHS financial goals. Maintains control of established positions and manages the recruitment and retention of qualified staff. Maintains patient/employee confidentiality in the management of information. Evaluates nursing staff performance and mentors, coaches, counsels and disciplines staff as needed. Monitors availability and maintenance of nursing equipment and supplies. Conducts regular staff meetings to ensure communication with staff about progress, changes and compliance with all hospital policies and procedures. Encourages, recognizes and rewards staff participation in quality and safety initiatives, compliance and successes. Facilitates organizational change initiatives by using effective strategies. Leads a shared decision-making model by establishing a structure of shared governance, just culture and staff engagement in practice councils, departmental meetings and initiatives. Interprets and implements collective bargaining agreements affecting staff. Responds to protest of assignment in a timely manner as detailed in the collective bargaining agreement and discusses the resolution in unit staff meetings. Professional Development Performs as a clinical leader who actively creates and supports nursing practice and an environment of care that reflects clinical excellence. Organizes the environment of care to support clinical colleagues, patients and their families and members of the healthcare team. Projects and cultivates a professional image to colleagues by communicating caring, respect, compassion, empathy and trust. Incorporates ethical principles into decision making for patient and family in collaboration with the clinical nursing staff and the healthcare team. Fosters a spirit of scholarship, inquiry, life-long learning and innovation for self and others. Acquires and disseminates knowledge and skills relative to the role, patient population, clinical specialty and global or local health community needs. Leads shared decision making through unit/service practice councils, nursing department and hospital committees, and clinical service/unit initiatives. Serves as a professional role model and preceptor for new leadership staff and students. Integrates the values of research and evidence-based practice and its application to nursing practice and the environment of care. Contributes to the profession of nursing through leadership and participation in professional organizations. Qualifications Bachelor of Science with a major in nursing required, master's in nursing or health related field preferred. 3 years minimum Emergency Department (ER) clinical nursing experience with some Charge Nurse/Supervisory experience preferred. Must have relevant clinical competence in area of nursing practice assigned. Licensed as a registered nurse with current registration in New York State. ACLS, BCLS, PALS from AHA required. Overview Carries out patient care responsibilities with emphasis on decision-making related to the steps in the nursing process to include assessment, diagnosis, and identification of expected outcomes, planning, implementation and evaluation. As a member of the interdisciplinary healthcare team, formulates the initial plan of care with the provider (physician or nurse practitioner) and collaborates with them to implement a comprehensive patient-centered plan of care to achieve expected outcomes; utilizes principles of assignment and delegation consistent with the NY State Nurse Practice Act. Responsibilities Provides professional nursing care to patients within unit. Diagnose and treat response to actual or potential health problems by means of nursing techniques and methods such as case finding, health teaching, health counseling and the provision of care supportive to and restorative of health and well-being. Maintain safe levels of care while adhering to a medical regimen. Collaborates with the physician and other health care providers to assure that a goal oriented care plan is consistent with medical regimen and hospital procedures. Performs other related duties as instructed. Qualifications BSN with three (3) years’ experience preferred. Current NYS license to practice as a Registered Professional Nurse. BLS Required. Description Medical Assistant-Family Healthcare-Full Time-Day-MSQ The Medical Assistant is an individual who functions in a support role to Clinical Nurse Manager by providing direct patient care and related activities as assigned. As a medical assistant one must be able to multi-task while at the same time focus on details of the proper identification of patients to ensure their safety, label all specimens correctly in accordance with the hospital policy and procedures established by the laboratory. Supports the mission, vision and philosophy of the institution. Responsibilities Assist the primary care provider and specialists with the physical examination of the patients as needed. Escort patient to the examination room, instruct patients on the preparation for examination. Assist in the disrobing and dressing after examination is completed if necessary to ensure patient safety and comfort. Responsible for the accurate recording of the patient vital signs; heart rate, blood pressure, respiration and weight of patient. Order and restock supplies in all patient examination rooms. Rotate and check all the examination rooms for expiration of supplies. Maintain the condition of the examination rooms between patient visits for cleanliness as well as necessary supplies to ensure JCAHO and DOH compliance. Perform venipuncture, including preparing necessary supplies for blood drawing, verifying patient identification and observing blood drawing procedures and protocol at all times. Make appropriate copies of EKG tracing if needed. Distribute original tracings and copies according to established hospital policy. Instruct patient on the collection of laboratory specimens , i.e. urinalysis, cultures, 24 hour specimen collection. Be responsible for the labeling of these specimens. Assist in the maintenance and care of departmental equipment for preventative maintenance. Maintain electrocardiogram and laboratory supplies, re-order and re-stock areas as needed Assist in the maintenance of the medical records. Prepare and maintain relevant department records and statistics. Coordinate referrals to specialists. Coordinate medical clearance for all pre-surgical patients. Maintain a monthly log of surgical patients. Maintain a monthly log for pap smears and mammography. Assist with preparation of lab slips, specialty appointment and other patient paperwork if required. Maintain established departmental policies, procedures, objectives, quality improvement program, safety standards and infection control procedures. Daily check if the refrigerator temperature log. Daily check that specimens are picked up in accordance with courier service schedule. Check at end of shift that all specimens were picked up. Assures and maintains a safe, uncluttered clean environment for patients, staff and others. Maintains attendance, punctuality and dress code in accordance with departmental policies. Demonstrates knowledge and adherence to Hospital and Departmental Policies and Procedures. Perform all other duties required. Qualifications Education: High school Diploma. Completion of a Medical Assistant certificate program from an approved school or two years of experience working as a medical assistant.
