The primary purpose of this position is the application of clinical knowledge to coordinate member care needs for assigned members who are primarily medically complex and require intensive medical and psychosocial support. The Care Coordinator works directly with members to ensure appropriate access to services and providers. Coordination of member care needs across the continuum begins with early identification of health risk factors, education, and prevention using the six essential activities of case management as per CMSA. Coordination also assures the provision of appropriate services in acute, home, chronic and alternative care settings, to meet member needs in the most cost-effective manner available. Primary Responsibilities Application of clinical knowledge to effect efficient case management for those accessing their Medicare/Medicaid benefits Incorporates evidence-based clinical practices into care coordination activities Delivers member-centered, individualized self-management support and patient education Installs appropriate equipment or supplies as needed and educates member on safe and effective use Facilitates and participates in group visit strategies as defined by site, member and/or provider needs Ensure appropriate utilization and consistent application of the health benefits Conduct telephonic and face to face assessments according to Health Plan policy and procedure, including CommunityCare System documentation and State prescribed timeframes Serve as member advocate and facilitator to resolve issues that may be perceived as barriers to care Provide education and coordination of community resource referrals to members and providers Develop, implement, maintain and evaluate a member centered, individual care plan when services are indicated, based on member care needs identified through proactive collaboration and communication with members, families and providers Collaborate and communicate with other members of the Care Coordination Team to improve the quality and efficiency of health care delivery and assure smooth transition of care coordination activities to other team members Coordinates care with UnitedHealthcare Inpatient Care Manager when member becomes hospitalized or seeks Emergency Room Services Monitor hospital encounters of assigned members and provide education and/or interventions as necessary to reduce frequency Refer clinical decisions beyond level of authority for members who do not meet established criteria to manager and/or Team Leader for review and decision Participate in the Health Plan Quality Improvement process including recognition of quality-of-care issues and forward information to appropriate staff for review and resolution Utilize Health Plan software to enter service requests, document, and monitor all aspects of patient care coordination and service delivery Maintain time sensitive documentation to assure compliance with regulatory agencies Coordinate services for members following Health plan policy Perform other duties as assigned You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications Current nursing license (RN) OR Licensed Social Worker 2+ years of clinical experience as an RN or Social Worker 1+ years of experience with MS Office, including Word, Excel, and Outlook Valid driver's license, access to reliable transportation and the ability to travel up to 75% within an assigned territory to meet with members and providers Reside in or within a 50-mile radius commutable driving distance to Coffee, Franklin, Van Buren, Warren or Grundy Counties Preferred Qualifications BSN, Master's Degree or Higher in Clinical Field CCM certification 1+ years of community case management experience coordinating care for individuals with complex needs Long term care experience Experience working in team-based care Working knowledge of Medicare/Medicaid regulations Working knowledge of medical terminology Soft Skills Excellent organization, communication and time management skills All Telecommuters will be required to adhere to UnitedHealth Group's Telecommuter Policy. Position will be 7 days on 7 days off We are seeking a dedicated Afterhours RN Care Manager to provide high-quality, compassionate hospice care outside standard business hours. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Clinical administrative responsibilities, workflow auditing, and survey readiness support are also part of this role. Essential Functions Ensure appropriate delivery of hospice services to eligible patients and families. Provide direct in-person patient care during on-call, weekend, and after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges. Conduct initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs. Support the IDG process through chart reviews, plan of care updates, and interdisciplinary communication. Develop and revise the Hospice Plan of Care in collaboration with physicians and IDG members. Educate patients and families on disease progression, symptom management, medications, and hospice philosophy. Communicate significant changes in patient condition promptly to hospice and attending physicians. Maintain accurate, timely clinical documentation including assessments, care plans, and notes. Assist with rescheduled, missed, or reassigned visit requests based on triage direction. Participate in calendar and scheduling modifications, including service codes and clinician assignments. Support administrative and compliance activities including workflow audits, survey readiness tasks, and documentation collection. Assist with TPE reviews, QAPI efforts, and regulatory compliance. Ensure infection control standards and universal precautions are consistently followed. Provide leadership and guidance to field staff; conduct supervisory visits when appropriate. Participate in required meetings, after-hours updates, and trainings. Maintain proficiency in pain and symptom management for hospice and end-of-life care. Transportation When required and authorized, caregivers may: Transport clients to appointments. Assist with approved errands. Transport clients to social activities. Help clients enter and exit the vehicle. Secure mobility equipment appropriately. Follow all client-specific transportation instructions. Maintain a clean, safe and insured vehicle. Report any transportation concern immediately. Caregivers may only transport clients when the assignment has been approved and all transportation requirements have been satisfied. Night and Overnight Shift Expectations Caregivers working night or overnight assignments must remain fully awake, alert and attentive throughout the entire shift. Nighttime responsibilities may include: Completing scheduled safety checks Monitoring the client’s condition Assisting with toileting and incontinence care Repositioning the client Providing medication reminders when authorized Responding immediately when the client needs assistance Completing approved nighttime housekeeping responsibilities Maintaining a clean and safe environment Preparing the client’s environment for the morning routine Documenting care and observations in real time Sleeping, napping, dozing, lying down to rest or closing your eyes for extended periods is strictly prohibited, even when the client is sleeping. Failure to remain awake and attentive may result in immediate removal from the assignment and additional corrective action. Physical Requirements Caregiving can be physically demanding. Attendance and Communication Expectations Dependability is essential in home care. Caregivers must: Arrive on time for every accepted assignment. Confirm assignments according to company procedures. Maintain reliable telephone communication. Respond promptly to scheduling messages. Notify the office immediately if an emergency affects attendance. Follow all call-off procedures. Clock in and out from the approved client location. Remain with the client until the scheduled end of the shift. Remain until properly relieved when continuous care is required. Never leave a client unattended when supervision is required. Never leave early without office authorization. Notify the office if a client or family asks them to arrive late, leave early or change the scheduled hours. Complete all required documentation before the end of the shift. Failure to report for an accepted shift, leaving a client without authorization or failing to follow safety procedures may result in removal from the assignment or additional corrective action. Documentation Requirements Caregivers must document services accurately and in real time. Required documentation may include: Completed care-plan tasks Personal care provided Meals and hydration Medication reminders Toileting and incontinence care Repositioning Blood glucose-related support Client behavior and mood Changes in condition Skin observations Incidents Office notifications Transportation Incomplete tasks and the reason they were not completed Caregivers must never mark a task complete when it was not performed. Background and Screening Requirements Applicants must: Complete a comprehensive background check covering up to 15 years, where legally permitted. Complete required abuse, neglect and sex-offender registry checks. Complete a pre-employment drug screening when applicable. Participate in random drug screening when required by company policy. Provide complete and accurate application information. Report criminal charges or legal changes that may affect their ability to provide safe care. Have no disqualifying history that would prevent safe employment, subject to applicable laws and individual review. ACLS, PALS, Basic Arrhythmia, Moderate/Deep Sedation and TNCC preferred. Experience: One year of experience in an acute health care setting preferred. Skills: Nursing skills as defined in the Law Governing the Practice of Nursing in Georgia.
