Follow the care plan after assessing the patient’s needs. Keep clear and timely records of the care provided. Teach and support patients and their families about the illness and care plan. Help coordinate care by communicating with the hospice team (like aides, chaplains, social workers). Manage pain and symptoms, checking how well treatments are working. Three years nursing experience. Community health/hospice or medical/surgical experience is preferred. Currently licensed as an LPN/LVN in the State. Comply with accepted professional standards and practice. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Possess and maintain current CPR certification. Understand philosophy of hospice concept. Also understand needs of the terminally ill. Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems. Demonstrate good verbal and written communication, and organization skills. Key Responsibilities Adhere to care plan to ensure safe and competent care for the patient. Complete visit reports promptly and accurately within the electronic medical record system. Confirm daily visit schedules to ensure seamless coordination with other staff. Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required. Understand and adhere to the agency’s process for managing and addressing customer complaints. Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies. Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge. Qualifications Must have a CNA or HHA license. Must have completed a minimum of 75 hours of training, which includes an introduction to personal care services. Must be free from health problems that may be injurious to patient, self, and coworkers and must present appropriate evidence to substantiate this. Ability to comprehend the basics of personal care, housekeeping, and meal preparation. Must understand and respect patient needs including ethics and confidentiality of care. Must have a valid driver’s license and access to a reliable and insured vehicle. Ability to visit homes that may not be wheelchair accessible. Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
