Summary : The RN Clinical Nurse (Ambulatory) provides patient-centered, specialized, evidence-based nursing care across the continuum through an interprofessional approach to treatment, research, education, and advocacy. Contributes to the goals of the department by being accountable for the delivery of compassionate and safe care within the scope of practice as defined by the Wisconsin Board of Nursing and ThedaCare policy. Through collaborative practice with members of the care team, is responsible for patient outcomes that meet the high quality of care provided by ThedaCare. Furthers the professional practice of nursing at ThedaCare by promoting a culture of innovation and a commitment to growth and professional development. Job Description: Schedule Variable Monday through Friday, between 7:00am and 6:00pm 8 - 9 hour shifts No weekends No holidays KEY ACCOUNTABILITIES: Utilizes the nursing process, evidence-based practice, and specific competencies to assess the physical condition and nursing needs of patients, and develops a plan of care in a collaborative practice with the patient and interprofessional team. Plans for the care needs of the patient in collaboration with the interprofessional team to provide the highest quality of care and clinical outcomes. Demonstrates clinical expertise in the provision of care in the clinical specialty assigned, and performs all functions of the professional clinical nurse (RN), which are age appropriate, developmentally sensitive, and culturally specific. Identifies ways to improve the patient’s experience of care, streamline care processes, and lower costs while promoting quality to improve patient, family, and team member satisfaction. Contributes to a professional environment that encourages mentoring, engagement, and development to retain expert clinicians. Provides consultation and maintains positive relationships with physicians and other interprofessional team members, collaborating to problem solve and improve patient care. Demonstrates, anticipates, and proactively manages risk to prevent crises. Performs skillfully in life threatening emergencies, matching demands and resources during crises situations. QUALIFICATIONS: Bachelor of Science in Nursing (BSN) preferred Associate's Degree in Nursing (ADN) required Current Wisconsin RN Licensure American Heart Association Healthcare Provider Basic Life Support (BLS). PHYSICAL DEMANDS: Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of Fifty (50) pounds without assistance Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties • Manual dexterity and hand-eye coordination to perform patient care procedures WORK ENVIRONMENT: Frequent exposure to sharp objects and instruments Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock Occasional high noise level in work environment Standing and/or walking for extended periods of time Transporting, transferring, positioning patients and/or equipment from one location to another; little likelihood for injury if proper body mechanics and procedures are followed Possible exposure to communicable diseases, hazardous materials, and pharmacological agents Occasional contact with aggressive and or combative patients. This position involves frequent phone communication with patients and staff. All inbound and outbound calls are recorded and monitored for quality assurance, training, and compliance purposes. Responsibilities: Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs for after-hours calls. Provides a complete physical assessment and history of current and previous illness. Available to receive and respond to calls from Triage during assigned shifts making visits to patients with changing conditions, and completing week end admissions. Provides professional nursing care by utilizing all elements of nursing process. Assesses and evaluates patient’s status by: Writing and initiating plan of care Re-evaluating patient and family/caregiver needs Participating in revising the plan of care as necessary Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. After hours on-call responsibilities included remaining available for all calls related to patient care and making all appropriate visits as necessary. Remaining with patient and or family through a crisis period until resolved and or relived. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Communicates with community health related persons to coordinate the care plan. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. Provides and maintains a safe environment for the patient. Assists the patient and family/caregiver and other team members in providing continuity of care. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. Attends interdisciplinary group meetings. Ensures that arrangements for equipment and other necessary items and services are available. Supervises ancillary personnel and delegates responsibilities when required. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. Fulfills the obligation of requested and/or accepted case assignments. Actively participates in quality assessment performance improvement teams and activities. Job Description: SCHEDULE: (1) shift every 2 weeks required May pick up open shifts as available No weekend or holiday shifts required UNIT DETAILS (Cardiovascular-Appleton Hospital): The Cardiovascular Unit is located at ThedaCare Appleton which is part of a highly Accredited STEMI (ST-elevation myocardial infarction) program. We are a twenty four (24) bed telemetry monitored unit that cares for a broad spectrum of Cardiovascular patients. Patient care include hemodynamic monitoring, treatment of adult patients during the recovery phase from cardiovascular and thoracic surgery, STEMI, exacerbation and treatment of end stage heart failure and the treatment of cardiac arrhythmias. This unit also serves as the stroke unit for the hospital as well. KEY ACCOUNTABILITIES: Performs delegated tasks using evidence-based practice and specific competencies to collect data on the physical condition and nursing needs of patients, and assists with implementing the plan of care. Assists in meeting the care needs of the patient in collaboration with the interprofessional team to provide the highest quality of care and clinical outcomes. Demonstrates clinical expertise in the provision of care in the clinical specialty assigned and performs all functions of the licensed practical nurse that are age appropriate, developmentally sensitive, and culturally specific. Identifies ways to improve the patient’s experience of care, streamline care processes, and lower costs while promoting quality to improve patient, family, and team member satisfaction. Contributes to a professional environment that encourages mentoring, engagement, and development to retain expert clinicians. Maintains positive relationships with physicians and other interprofessional team members, collaborating to problem solve and to improve patient care. Demonstrates, anticipates, and proactively manages risk to prevent crises. Performs skillfully in life threatening emergencies, matching demands and resources during crises situations. QUALIFICATIONS: Current Wisconsin LPN Licensure American Heart Association Healthcare Provider Basic Life Support (BLS). PHYSICAL DEMANDS: Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of Fifty (50) pounds without assistance Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties WORK ENVIRONMENT: Frequent exposure to sharp objects and instruments. Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock Occasional high noise level in work environment Standing and/or walking for extended periods of time Transporting patients and/or equipment from one location to another; little likelihood for injury if proper body mechanics and procedures are followed Possible exposure to communicable diseases, hazardous materials, and pharmacological agents Occasional contact with aggressive and or combative patients.
