Penn Medicine Princeton Health (PMPH) is seeking a per diem RN (Registered Nurse) to work the day shift within Princeton House Behavioral Health (PHBH). This position is within Behavioral Health Float. The RN will be floated to multiple units. Our inpatient psychiatric services provide short-term stabilization and treatment for mental health disorders. We provide care in a secure calm environment where patients can focus on healing. Daily psychiatrist visits and around-the-clock medical supervision help patients on their journey to better mental health. Location: Princeton House Behavioral Health (905 Herrontown Road Princeton NJ). Key Roles and Responsibilities: Develops and maintains a plan of nursing care based on goals derived from identified needs; revises the patients individualized plan of care as indicated Implements individualized nursing plan of care, consistent with established policies, procedures, and protocols Evaluates and documents patient responses to nursing care, and participates in evaluating and improving the delivery of care on the unit Demonstrates accuracy in entering medication orders on the computer and medication administration Provides appropriate coordination of care and intervention to patients undergoing a psychiatric or medical emergency Scheduling Requirements (Tier 1): The day shift is considered 7am-3pm Work a minimum of one weekend per 4-week schedule (Consecutive Saturday/Sunday) Two holidays per year (1 minor and 1 major) One difficult to staff days per year What We Require for this Role: Education Bachelor’s degree required, or in pursuit of A registered nurse with an Associate’s Degree with greater than 5 years of clinical experience in a clinical specialty may be considered. Related Work Experience 15 months of RN experience required Psych experience preferred. LICENSURE, CERTIFICATION OR REGISTRATION REQUIREMENT Basic Life Support (BLS) required. Attends all assigned working neurology conferences, facilitating the conference with physician leader. Provides individualized patient/family information, support, education and resources regarding neurological conditions, diagnostic testing, procedures and surgeries. Serves as a consistent contact for patient/family throughout the process of diagnosis and treatment. Empowers patient/family to participate in the plan of care, documents compliance and concerns, monitors patient progress, consults with physicians and other members of the health care team at agreed upon intervals. Appropriately refers/assists with coordination of appointments for consultations, physician referrals, and community resources as related to the neurology program. Interacts with administration, manages and facilitates program improvement, keeps team up to date with program and monitors progress in achieving desired outcomes. Manages the cross continuum care of multidisciplinary patients to provide high quality of care in an effective and efficient manner with close attention to service excellence. Acts as network-wide neurology resource nurse and program coordinator of neurology working group. Identifies patient and/or provider needs and ensures delivery of quality services to patients. Collaborates with Network Clinical Studies Coordinator in clinical trials enrollment and data collection. Reviews and identifies appropriate cases for clinical trial eligibility with the study coordinator. Other related duties as assigned. PHYSICAL AND SENSORY REQUIREMENTS: Sit up to 7 hours per day; 3 hours at a time. Stand for up to 3 hours per day; 2 hours at a time. Walk 1 hour per day; 10 minutes at a time. ESSENTIAL DUTIES AND RESPONSIBILITIES: · Ensures that the facility is in compliance with Federal and PA regulations as well as the organization’s policy and procedures. · Conducts audits of the facility's systems and evaluates for survey readiness, compliance with Medicare requirements and optimal quality of care for each resident. · Performs regular review of Electronic Medication Administration Records (EMAR), Point of Care (POC), Point Click Care (PCC) to ensure compliance. · Regularly monitors wound care program in order to promote quality outcomes provided in the facilities to ensure appropriateness and effectiveness. · Reviews Risk management portal to ensure that incident investigation and follow up are completed timely. incident reports and follow up notes regularly to ensure state reportable incidents are being reported, proper procedure is being followed and documented. · Facilitates company policies and procedures related to patients and employees · Attends QAPI, monthly Infection Control, weight and psychotropic meetings · Ability to support the Director of Nursing and Administrator with day-to-day operations · Provides mentoring and support for facility administrative staff and assists with problem solving as needed · Reviews medical records prior to any legal requests for documents · Attends meetings as required; works cooperatively with members of the management and care team, maintaining effective interdepartmental and interpersonal relations with residents, staff, and visitors. · Assist in hiring, training, planning assigning and directing the work of facility staff as needed. · Conducts audits including resident charts, medication room and QA interventions. · Performs comprehensive assessment on residents. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required. · Bachelor’s Degree in Nursing required. · Five years management experience with experience in long-term care; Previous experience as a Director of Nursing Services (DON) preferred · Must possess a current, unencumbered, active license to practice as an RN in Pennsylvania) · Proficient in communication and writing skills · Must have a valid driver’s license., position requires travel · General knowledge of professional basic nursing, restoratives and personal care services. · Ability to read, and interpret documents, write reports, and correspondence and possess · High level of interpersonal skills to handle sensitive and confidential situations and documentation, and interaction with coworkers.
