Department 10307 Pediatric Intensive Care Unit Pay Range $33.08 - $60.18 Job Description Summary $25,000 Bonus with a 3-year Commitment Day Shift / Night Shift Rotation Weekend and Holiday Rotation Who We Are The Pediatric Intensive Care Unit (PICU) is a 46-bed unit. We will be expanding to 60 beds in 2027! This unit serves as a regional referral center and is a designated Level I Trauma Center for comprehensive care to pediatric patients and their families. The PICU Patient Care Management Team coordinates interdisciplinary patient assessment and patient care and provided by pediatric intensivists and specialty nurses in conjunction with multiple specialty services within the Medical Center. Patients are admitted to the PICU from the ED, Trauma Services, the Operating Room, and transfers from in-house units as well as transports from other institutions via the Pediatric Transport team. Trauma patients comprise about 10%-15% of patients in the PICU. These patients often have multiple injuries or severe head trauma. The PICU is also a Pediatric Burn Center and receives referrals from the region. Other complex patients requiring general surgery, ENT, craniofacial, neurosurgical and other surgical subspecialties are also cared for in the PICU. Medical patients comprise 50%-60% of patients and include children with respiratory failure, meningitis or seizures, sepsis, multi-system organ failure and chronically ill children. Children with cancer also require PICU care for complications that develop as part of the underlying disease, chemotherapy or following bone marrow transplantation. Services are provided daily, 24 hours per day. The Division of Critical Care Medicine (CCM) provides services and care to all patients admitted to the Pediatric Intensive Care Unit (PICU). Specifically, care is provided for all patients in the PICU, patients within the hospital by request, patients within the hospital as part of all emergencies and resuscitations, and patients in the ED for “medical alerts” and “trauma stats”. The scope of services includes resuscitation, life support, monitoring and treatment for life threatening events and diagnostic issues related to these items. Patients may be admitted to and discharged from the PICU based on pre-established guidelines. See section on critical care units. The department utilizes a multi-disciplinary approach to care, utilizing the services of physicians, nurse practitioners, nurses, respiratory therapists, social services and other support services necessary to provide quality care. Attending physicians providing care are board eligible or certified in Pediatric Critical Care. The PICU provides CAT (Rapid Response Services) and Code Blue team leadership to support in house clinical emergencies. An attending or fellow along with an ICU nurse and Respiratory Therapist respond to assist in clinical rescues of in-house patients. This service can also be activated by a concerned parent. About The Role A Registered Nurse (RN) in the Pediatric Intensive Care Unit (PICU) provides specialized, high-acuity care to critically ill and injured infants, children, and adolescents requiring continuous monitoring and advanced life support. Working within a 46-bed regional referral and Level I Trauma Center, the PICU RN collaborates closely with pediatric intensivists, nurse practitioners, respiratory therapists, and multidisciplinary specialty teams to assess, plan, implement, and evaluate patient care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Child Psychiatric Unit The inpatient Child Psychiatric Unit is a 12-bed unit. It is a short-term child oriented program designed to provide crisis intervention and stabilization to children 3-13 years of age. The average length of stay is three to seven days. This individual will assess physical, psychosocial, cognitive, and functional components, develop a treatment plan, order and interpret diagnostic components of the plan, and prescribe non-pharmacological and pharmacological agents. The Nurse Practitioner functions professionally under a practice agreement with the supervising physician and operationally under the AVP of Clinical Operations and/or the designee. Essential Functions & Responsibilities: In collaboration with physician, demonstrates ability to counsel patients and family members about diagnosis, testing, and risk of procedures. Performs comprehensive health assessments for the purpose of diagnosing and treating individuals. Formulates appropriate differential diagnosis, develop treatment plan to include appropriate diagnostics; identifies non-pharmacological interventions, appropriate pharmacological agents and education. The plan is based on mutually established priorities to maximize the heath potential of the patient. Provides relevant patient and family education; makes appropriate referrals to other professional and community agencies. Orders and provides preliminary interpretation of diagnostic studies. Prescribes non-pharmacological therapies and pharmacological agents within the defined scope of service and according to guidelines established by the supervising physician. Demonstrates the ability to use effective organizational/prioritization problem-solving and decision-making abilities in practice. Provides emergency treatments and initiates emergency life saving measures per ACLS guidelines. Meets all the requirements of the practice agreement with the supervising physician under the regulations and requirements of the Commonwealth of Virginia. Delineation of Privileges: Obtain, record and dictate medical histories Perform, record and dictate physical examinations Make daily rounds, record progress notes, write routine comfort and diet orders, order laboratory tests and non-invasive examinations, write approved routine orders, and change dressings Prepare patients for discharge from the hospital Dictate discharge and transfer summaries Evaluate patients who require a cardiology consult Clinical procedures (as necessary or applicable): Apply and change dressings Remove sutures and staples Perform venipunctures Insert IV needles and catheters Communication: Educate patient/family regarding disease, procedures, medication, nutrition, healthcare, and post-discharge care Dictate letters to physicians, patients Act as liaison between patients and physicians Act as liaison between nursing and physicians Request consults from health agencies and act as liaison Make and receive phone calls to/from patients, physicians and nurses for information, advise, triage Other Maintain statistics Complete insurance forms and home health forms Maintain written material; teach devices and special equipment for use of personnel and patients. Qualifications: Registered Nurse licensed to practice in Virginia or eligible, required MSN, required Current Virginia state certification as an Adult Nurse Practitioner, required AHA BLS Provider CPR required Recent adult medical-surgical experience (5 years of experience), preferred Ability to work effectively in emergency situations and under stress Proficient leadership skills Ability to work cooperatively/collaboratively as part of a team, developing productive working relationships with diverse groups of people.
