Evaluates patient need for hospital and extended care resources (Medical Social Work, Pastoral Care, rehabilitation care, long term care, home health care, and community resources) and when appropriate, makes referrals Acts as a liaison between patients, physicians, ancillary and community services throughout the entire patient experience from diagnosis to post-discharge to ensure effective healthcare management and delivery of transitional services. Develops, implements, coordinates and communicates the plan of care encompassing acute phase through transition out of acute care. Builds and maintains strong collegial relationships with physicians, nursing team and leaders to provide quality of care. ● Coordinates care using Pathways or Plan of Care and takes responsibility in the ongoing development and revision of Pathways and Plan of Care. Participates actively in assigned groups and committees. Ensures appropriate use of community and outpatient resources to adequately support care needs after discharge Manages and coordinates appropriate discharge plans to ensure LOS appropriate for care needs this includes ensuring and facilitating the achievement of quality, clinical and financial outcomes, negotiating, procuring, and coordinating services and resources needed by the patient/family, and intervening at key points for individual patients. Evaluates outcomes related to the Case Management process including LOS, Readmission reports, patient satisfaction and financial variances related to case management participation in the patients care. Investigates problems and complaints from patients, visitors, physicians, and staff. Implements appropriate action and follow-up. Initial contact for atypical events. Conducts initial assessment of event. Initiates appropriate action, ensuring stabilization of patient. Ensures notification of risk management, CSI, and department manager/director. This is an annual CBL that describes the difference in a swing bed patient versus an in-patient or observation patient. The Case Manager will understand the documentation differences and the specific care required for each individual swing bed patient. We are seeking a dedicated Hospice Nurse Practitioner to perform Face-to-Face (F2F) encounters essential for hospice eligibility certification and recertification. In this role, you will gather comprehensive clinical information, perform detailed assessments, and collaborate closely with hospice physicians and interdisciplinary teams to ensure quality patient care and regulatory compliance. Key Responsibilities: Perform and complete physical assessments for hospice F2F encounters within CMS-established regulatory timeframes. Document clinical findings accurately and attest F2F encounters in patient medical records. Utilize Local Coverage Determination (LCD) criteria per hospice policy and regulatory requirements. Communicate clinical findings, review treatment plans, lab results, diagnostics, and medications with certifying physicians. Collaborate effectively with physicians, interdisciplinary group (IDG) members, patients, caregivers, and families. Provide clinical recommendations to the IDG based on F2F assessment findings. Conduct assessments in the patient’s place of residence, including home, skilled nursing, or other settings. Notify the hospice agency promptly if unable to complete F2F visits as scheduled. May provide F2F coverage for multiple provider numbers with approval. About the Role We are currently looking for an experienced Medical Assistant to join our team. The Medical Assistant is a key component of our care team and works closely with a primary care physician to deliver excellent patient care and provide an exceptional guest experience to our patients. Because of our unique model, our Medical Assistants oversee the full spectrum of the patient experience- acting both as a Medical Receptionist as well as assuming clinical responsibilities of a Medical Assistant. Essential Duties and Responsibilities The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all-inclusive. Escorts patients to room and ensures that exam rooms are thoroughly cleaned and stocked Reviews previous medical information and gathers any relevant updated health information from the patient to inform the provider Take vitals, perform blood draws, give injections, bandage wounds, assist with procedures. Work with the contracted labs for additional tests Prepares and administers approved medications and immunizations via oral, topical, inhaled, intramuscular, subcutaneous or intradermal at the direction of and upon written order from the Provider Partners with other members of the care team to develop individual patient plan of care including identifying and working to address gaps in care Front desk duties, including: patient scheduling, medical record requests and abstraction, documentation, coordinate specialist visits, referrals, and other care outside of the clinic, check-in, check-out, etc. Records accurate and pertinent data in the medical record according to documentation guidelines Process test results and provide the patient with timely results via e-message, mail or phone as directed in written instructions by the Provider Participates in outreach to patients to drive engagement among eligible patient groups Maintains clinical and office supplies and equipment for treatments About You Minimum of 1 year of experience working as a Medical Assistant, preferably in Primary Care. May vary based on clinic needs Graduation from a formal Medical Assistant program or other related program National or state-specific MA certification or registration is strongly preferred. MA certification or registration is required for employment in States where certification/registration is required.
