In addition to base salary, this role is eligible for additional compensation, including bonus opportunities and a comprehensive benefits package. Responsibilities Work closely with patients’ medical providers to develop and continually adapt care plan Perform in-home care management visits to execute care management plan Monitor biometric data and follow approved protocols for any necessary interventions Inventory and reconcile medications and coordinate with pharmacists and prescribers Perform patient health assessments and surveys as required Deliver individual and group education on CKD, ESRD, dialysis and associated comorbidities Encourage medication and treatment adherence through frequent contact with patients Engage family and social support groups in the education and care of patients Serve as the primary point of contact and be the first call when patients have questions (business hours) Provide education and coaching around medications, medical conditions, diet, exercise, and lifestyle choices Educate patients and facilitate conversations around proactive care decisions, especially relating to Advance Care Plans and ESRD treatment modalities Obtain vital signs when visiting patient and escalate any concerns to the provider Initiate patient relationships through enrolment and onboarding processes Perform post-op and hospital discharge visits to help patients through vulnerable transitions Review and document patient updates and progress in care management platform Coordinate with dialysis providers to ensure transitions of care are seamless Position Requirements Frequent local travel to perform in-home visits Basic Life Support (BLS) certification is required in this role. The registered nurse/ license vocational nurse plans, organizes and directs hospice care services specific to each patient/family which includes instruction and evaluation in the home. The professional nurse is accountable to the Director of Patient Care Services for implementing the patient plan of care and communicating each patient needs to the Interdisciplinary team. The professional nurse is also responsible for communication and collaboration with community physicians as well as family members regarding individual patient care. The Hospice Team Nurse provides professional nursing care to patients by utilizing all elements of nursing process. 2. . Assesses and evaluates patient’s status 3. Initiates the plan of care and makes necessary revisions as patient status and needs change. 4. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 5. This position will be TUESDAY-SATURDAY 12PM-8PM. 6. This position will be PRN. 7. This position will be working in different service areas such as the following - Alameda, Contra Costa, Napa, Santa Rosa, Solano and Sonoma. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. The Hospice Team Nurse completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. POSITION QUALIFICATIONS 1. Graduate of a National League of Nursing accredited school of nursing. 2. Current licensure in State of CA and CPR certification. 3. Registered nurses / Licensed Vocational nurse shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN / LVN license. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 6. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. 7. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Certified Nurse Aide 2:15 pm-10:45 pm FULL TIME

POSITION DESCRIPTION : A nursing assistant responsible for providing routine nursing care in accordance with established policies and procedures and as may be directed by the Charge Nurse, RN Supervisor, Director of Nurses or Administrator, to assure that the highest degree of quality resident care can be maintained at all times. QUALIFICATIONS Currently enrolled in a school for nursing assistants or have a license as a certified nursing assistant. A minimum of 6 months experience as a certified nursing assistant or clinical experience while enrolled in a nursing assistant certification class. Must possess a sincere desire to serve the ill, aged and infirm. Ability to read, write and speak the English language. Possess the ability to deal tactfully with personnel, residents, family members, visitors, and the general public. Willingness to work harmoniously with other personnel. Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level they are currently functioning. Must be in good general health and demonstrate emotional stability. Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility. GENERAL DUTIES AND RESPONSIBILITIES: GENERAL Perform all duties as assigned and in accordance with facility’s established policies and procedures, nursing care procedures and safety rules and regulations. Make resident rounds at the beginning of each shift and every two hours thereafter to administer quality nursing care. Verify the identification of a resident before administering nursing care or serving food, to assure that appropriate care is being provided to the correct resident. Report missing ID bands to admission coordinator immediately. Make beds, clean bedside and overbed tables. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator.
