Illustrative Duties Graduate Nurse (T&RC) Prepares or assists with preparation of nursing care plans consistent with individuals’ comprehensive treatment plans and medical needs. Carries out treatment procedures, administers medications, and monitors individuals’ conditions. Records notes and maintains individuals’ medical records and nursing care plans, including nursing care, medications, and treatments, reactions to care and treatment, intake and output of liquids and solids, and vital signs. Delegates responsibility for appropriate nursing activities, skills, and/or procedures to nursing and direct care staff; and makes clinical rounds to assess individuals’ nursing care needs and observe and evaluate the provision of nursing services. May perform the full range of supervisory duties over Graduate Nurses (T&RC), Teaching & Research Center Licensed Practical Nurses, and direct care staff; provide information to Registered Nurse Supervisors 1 (T&RC); and prepare or contribute to the preparation of reports and performance evaluations for lower-level staff involved in nursing care activities. Registered Nurse 2 (T&RC) Positions perform duties in each of the three functional areas described below. Direct Individual Care: Provides nursing care to individuals in an acute care environment requiring complex nursing skills; uses advanced knowledge and skill in nursing assessment, planning, implementation, and evaluation activities. Observes and responds to individual emergency situations, and sets priorities for care based on individual acuity, preferences, and the possibility of effective intervention. Maintains a minimum standard of continuing education for specialty areas. Evaluates and assesses individual staff and nursing student development needs. Counsels and implements a variety of teaching strategies to meet those needs in consultation with nursing management staff. Participates in in-service and other educational programs related to clinical specialties. May identify, develop, and implement nursing research studies on clinical units, in consultation with medical staff consultants. Clinical Management: Consults and coordinates with other health care team members to resolve complex clinical issues and develop short and long-term clinical goals for assigned units. Integrates and evaluates clinical care knowledge specific to the needs of unit individuals; develops unit-specific policies and procedures in consultation with nursing management staff. Identifies and implements corrective actions to improve the quality of individual care; may oversee equipment testing and supplies. Leads and attends various hospital committees. Assists Registered Nurse Supervisor 1 (T&RC) staff with implementing and achieving unit operating changes; may take charge of a unit in the absence of a Registered Nurse Supervisor 1 (T&RC). Resolves individual relation issues. Identifies and reports issues related to individual care, operation, and quality outcomes. Identifies potential conflict situations and facilitates solutions. Registered Nurse Supervisor 1 (T&RC) Plans, directs, and supervises nursing services on an assigned unit, including direct supervision of Registered Nurse 1 (T&RC) and other staff. Directs the development and implementation of nursing care plans on a unit. Provides individual care, and functions as an educator and nursing services expert. Assesses work methods of nursing staff and provides feedback to improve the efficiency and effectiveness of care delivered. Coordinates medical and nursing care needs with other health care providers and departments. Ensures proper functioning of equipment and unit compliance with safety and environmental standards. Job Description The Department of Pediatrics at SUNY Downstate Health Sciences University is seeking a full-time TH Assistant Director of Nursing / Nurse Practitioner, Outpatient Gastroenterology Infusion Center. The successful candidate will: Provide advanced care, including administering intravenous medications, fluids and nutrients to children to treat chronic illnesses, infections, or for wellness purposes like dehydration and fatigue. Demonstrate responsibility for patient management assessing and educating patients and families, and monitoring patients before, during and after therapy. Oversee pediatric infusions, providing a higher level of care and diagnostic capabilities, including ordering necessary lab tests and adjusting treatment plans. Work both independently and under physician supervision, to create treatment plans, start infusions, monitor for reactions, and educate families. Perform patient assessments, manage medications, order diagnostics, and assist with procedures like endoscopies and colonoscopies. Provide assistance in coordinating, scheduling of the outpatient procedures and prior authorizations for infusions. Coordinate patient care for infusions and therapy. Document all care provided in the patient's medical record; completes all required patient care documentation and submit all documents to the office. Develop further and manage patient care panel with participation in managed care programs, and other direct patient care programs as applicable. Participate in and provide departmental education activities to medical students, residents, fellows and allied health professional students. Job Description Distinguishing Characteristics All positions are in the non-competitive jurisdictional class. Nursing Assistant 1: entry level; performs basic patient care duties under direction of a nurse, physician, or other clinician; may assist Nursing Assistants 2, physicians, and nurses in the performance of care functions. Incumbents at this level receive in-service training in a limited number of care procedures, leading to eligibility to qualify for appointment to Nursing Assistant 2. Nursing Assistant 2: full performance level; using detailed guidelines, standards, and procedures, and under the supervision of clinicians, observes patients and performs prescribed simple treatment procedures; attends to personal hygiene needs of patients; and performs limited cleaning and housekeeping duties directly related to patient care. Treatment tasks performed require only knowledge of a limited number of specific treatment procedures performed at the facility. Nursing Assistant (Certified): full performance level; performs duties similar to a Nursing Assistant 2, but functions are performed in a long-term care facility and require certification as a New York State Certified Nursing Assistant. Related Classes Rehabilitation Hospital Nursing Assistants perform similar functions to those of Nursing Assistants but are focused on rehabilitation activities and are only classified at DOH’s Helen Hayes Hospital. Collects and carries patients’ laundry and dry cleaning and delivers objects to appropriate locations in the health care facility. As directed by clinical staff, moves, positions, lifts and transports patients using a variety of lifts and other aids in order to make patients comfortable, prepare patients for treatment, deliver patients to treatment areas, and to prevent contractures. Brings bedpans and urinals to patients requiring assistance and attends to incontinent patients. Talks and listens to patients and responds by encouraging patients to cooperate and participate in treatment activities. May transport patients and supplies on occasion. Nursing Assistant 2 Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 1, but with greater proficiency. Nursing Assistant (Certified) Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 2, but also participates in required quarterly in-service education to maintain certification and meet all necessary requirements to be re-certified every two years. Communication Nursing Assistants have frequent face-to-face communication with patients and visitors for the purposes of conveying treatment procedure and visitor information. Incumbents attempt to encourage and support patients who may be despondent or apprehensive, and regularly communicate with clinical staff to convey information on patients’ physical conditions and responses to treatment.
