The Cardiac Nurse Navigator serves as a clinically expert, patient-centered coordinator across the continuum of cardiac care, functioning as a primary liaison between inpatient services, outpatient practices, procedural areas, and ancillary departments. This role facilitates seamless transitions, optimizes access, supports service line growth, improves outcomes, and enhances the patient and provider experience. The Cardiac Nurse Navigator works collaboratively with cardiologists, cardiothoracic surgeons, electrophysiologists, advanced practice providers, nursing leadership, care management, and administrative teams to ensure coordinated, timely, and evidence-based cardiac care delivery. Key Responsibilities Care Coordination & Continuum Management Coordinate patient care across inpatient units (primarily telemetry and CVU) procedural areas (cath lab, EP lab, OR), outpatient clinics, diagnostic services, and post-acute providers. Facilitate smooth transitions of care from hospital to outpatient follow-up, rehabilitation, home health, and specialty services. Collaborate with case management and social work to address barriers to care, adherence, and access. Patient Navigation & Education Provide individualized patient education regarding diagnoses, procedures, treatment plans, medications, and self-management strategies. Support patients and families through complex cardiac episodes (e.g., new diagnosis, structural heart interventions, advanced heart failure, arrhythmia management). Reinforce discharge instructions and ensure patient understanding of next steps in care. Serve as a consistent, trusted resource to reduce anxiety, improve engagement, and enhance satisfaction. Inpatient–Outpatient Liaison Function Act as the communication bridge between hospital-based teams and outpatient cardiology. Ensure timely transmission of clinical information, care plans, and procedural outcomes. Coordinate handoffs between proceduralists, hospitalists, cardiologists, and outpatient providers. Support the alignment of inpatient and outpatient workflows to reduce fragmentation, delays, and duplication of services. Service Line Support & Growth Enablement Support cardiac service line initiatives, including new program development, pathway implementation, and access optimization. Participate in multidisciplinary rounds, service line meetings, and program development committees. Assist with development and standardization of cardiac care pathways, protocols, and patient flow models. Identify gaps, inefficiencies, and opportunities for improvement across the cardiac care continuum. Quality, Outcomes & Performance Improvement Track and monitor patient progression across episodes of care. Support quality improvement initiatives related to readmissions, length of stay, patient experience, procedural throughput, and care transitions. Assist in data collection, audits, and outcome reviews related to cardiac services. Promote adherence to evidence-based practice, regulatory standards, and organizational policies. Physician, APP & Team Collaboration Build and maintain strong working relationships with cardiologists, surgeons, APPs, nurses, and ancillary departments. Support physician practices with coordination needs, patient access issues, and care navigation. Participate in patient case conferences and multidisciplinary care planning. Qualifications Current RN license in the state of practice. Bachelor of Science in Nursing (BSN) required; Master’s degree preferred. Minimum of 3–5 years of recent clinical experience in cardiac care (e.g., cardiology, cath lab, EP, CVICU, telemetry, heart failure, or cardiothoracic services). Strong clinical assessment, care coordination, and patient education skills. Demonstrated ability to work across departments and sites of care. Cardiac certification (e.g., PCCN, CCRN, CMC, CSC, CHFN) preferred. Experience in care management, navigation, or service line coordination preferred. MDS Coordinator RN Water’s Edge Rehab and Nursing Center at Port Jefferson is looking for a talented and hard-working MDS Coordinator to join our ever-growing team. We are seeking qualified candidates who have experience as an RN and are committed to help our patients and facilities receive the support they need. Responsible for completion of the Resident Assessment Instrument in accordance with federal and state regulations and company policy and procedures. Acts as in-house case manager by considering all aspects of the residents care and coordinating services with physicians, families, third party payers and facility staff. MDS Coordinator RN Essential Job Functions Oversees accurate and thorough completion of the Minimum Data Set (MDS), Care Area Assessments (CAAs) and Care Plans, in accordance with current federal and state regulations and guidelines that govern the process Acts as an in-house Case Manager demonstrating detailed knowledge of residents health status, critical thinking skills to develop an appropriate care pathway and timely communication of needed information to the resident, family, other health care professionals and third party payers Proactively communicates with Administrator and Director of Nursing to identify regulatory risk, effectiveness of Facility/Community Systems that allow capture of resources provided on the MDS, clinical trends that impacts resident care, and any additional information that has an affect on the clinical and operational outcomes of the Facility/Community Utilizes critical thinking skills and collaborates with therapy staff to select the correct reason for assessment and Assessment Reference Date (ARD). Captures the RUG score which reflects the care and services provided Demonstrates an understanding of MDS requirements related to varied payers including Medicare, Managed Care and Medicaid Ensures timely electronic submission of all Minimum Data Sets to the state data base. Reviews state validation reports and ensures that appropriate follow-up action is taken Facilitates the Care Management Process engaging the resident, IDT and family in timely identification and resolution of barriers to discharge resulting in optimal resident outcomes and safe transition to the next care setting Directly educates or provides company resources to the IDT members to ensure they are knowledgeable of the RAI process. Provides an overview of the MDS Coordinator and Assessor role to new employees that are involved with the RAI process. Teach and train new or updated RAI or company processes to interdisciplinary team (IDT) members as needed Analyzes QI/QM data in conjunction with the Director of Nursing Services to identify trends on a monthly basis Responsible for timely and accurate completion of Utilization Review and Triple Check Serves on, participates in, and attends various other committees of the Facility/Community (e.g., Quality Assessment and Assurance) as required, and as directed by their supervisor and Administrator MDS Coordinator RN Qualifications : Registered Nurse with current, active license in state of practice. Minimum two (2) years of clinical experience in a health care setting Minimum of one (1) year of experience in a long term care setting Prior experience as an MDS coordination accepted Training program available for RN candidates with demonstrated assessment skills Salary: Up to $140,000 a year Based on experience An Equal Opportunity Employer INDRN
