Overview Support Families & Patients in home setting 8:00AM – 8:00PM or 8:00PM – 8:00AM We are seeking a dedicated Continuous Care Licensed Practical Nurse (LPN) to join our team. Reporting to the Executive Director/Administrator or Director of Clinical Services, and working under the direction of a Registered Nurse, you will provide continuous nursing care to hospice patients in facility or home settings.
