Patient Education : Explain procedures to patients as part of the treatment team and provide comprehensive education on procedural information and post-care instructions. Coordination of Care : Coordinate the care of medically complex patients in collaboration with a multidisciplinary team to ensure comprehensive treatment plans. Post-Procedure Care : Oversee the post-procedure care of interventional radiology patients, including managing and treating any side effects. Order Management : Order necessary tests, procedures, and imaging to facilitate patient care and treatment outcomes. Minor Procedures : Potential to perform minor procedures and line placements as part of patient treatment. Qualifications Education: Master of Nursing Degree. The VAD Transplant Coordinator provides nursing care as an RN to Ventricular Assistive Devise and cardiothoracic transplant patients. Acts as the central point of contact to the Cardiothoracic Transplant Department for patients and their families who are managed with, or being considered for a VAD and cardiothoracic transplant. Provides off hours coverage to the service to handle patient phone calls, questions and troubleshooting. Provides in-patient and out-patient care, education and community resources for both VAD and cardiothoracic transplant patients. Educates the community about the program including, local Emergency Departments and EMS squads.
