The University of Iowa Health Care, Care Coordination Division is seeking a Nurse Clinician to function as a clinical nurse expert within the Emergency Department and care coordinator for the assigned patient population facilitating seamless transitions across the continuum of care. The nurse functions as an administrative and clinical liaison for individuals to optimize quality health care delivery using clinical assessment/screening tools, education reinforcement, follow-up on the plan of care recommendations and guiding the utilization of cost-effective resources. Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns) Ability to learn an electronic medical records system. Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area
