Weekend Clinical Triage Specialist RN, Connect to Care - Access Center

A Clinical Triage Specialist (CTS) (RN) - Connect to Care - Access Center will compassionately deliver an exceptional patient experience and provide clinical support to CTS-MA team members by serving as a clinical resource. The CTS-RN is responsible for using nursing judgment in answering/returning patient calls related to direct care provided by the practices. When appropriate, the caller’s symptoms will be assessed and triaged using approved nursing protocols and guidelines in order to assist in obtaining the appropriate level of care and/or self-care advice. JOB DUTIES AND RESPONSIBILITIES: Answers telephones, prioritizes clinical triage calls, follows clinical protocols, and coordinates services, as needed. Verifies patient demographic information and accurately enters the updated information into electronic health record. Serves as an escalation point for clinical patient issues and other POD team members requiring clinical support and provides clinical advice based on clinical protocols and procedures. Manages and responds to escalated electronic patient messages whenever not answering inbound patient calls, and uses clinical judgment to prioritize and accommodate patients. Creates a positive patient experience at every encounter, attempting to independently resolve any issues or concerns of the patient at the time of the phone call, within the scope of the role. Consistently meets productivity, schedule adherence, and quality standards as set by the Access Center. Utilizes all resources and guidelines at his/her disposal to effectively assess, prioritize, advise, schedule appointments, or refer calls when necessary to the appropriate medical facility or personnel. Accurately documents symptoms/complaints, nursing assessment, advice provided and patient/caller response. Partners with other Access Center teams/PODs and respective practice clinical team on behalf of the patient to assist with clinical concerns, medication refills, or scheduling appointments. Other related duties as assigned. PHYSICAL AND SENSORY REQUIREMENTS: Requires sitting for extended periods of time (up to 8 hours at time). Requires continual use of fingers, writing and computer entry. Requires ability to hear normal conversation and good general near and peripheral vision. BASIC SUMMARY The Registered Nurse (RN) provides skilled nursing services to home health patients in their place of residence. The RN follows the nursing process of assessment, planning, implementation, and evaluation, working in collaboration with the physician and interdisciplinary healthcare team to develop and update the patient’s plan of care. The RN is responsible for ongoing patient assessments, skilled procedures, education, documentation, and coordination of services, ensuring compliance with state and federal home health regulations. PRIMARY RESPONSIBILTIES/​ESSENTIAL JOB FUNCTIONS Assessment & Plan of Care 1. Performs comprehensive head-to-toe assessments of the patient's physical, psychosocial, emotional, family, support system, and environmental status for home care appropriateness. 2. Develops, implements, and evaluates an individualized plan of care in collaboration with the physician, therapists, social medical workers, and home health aides, ensuring adherence to medical and nursing diagnoses, patient needs, and standards of care. 3. Monitors and records vital signs, pain levels, and wound measurements on each visit. 13. Ensures proper execution and documentation of physician orders, treatments, and medication administration. 14. Communicates significant patient condition changes immediately to the physician and interdisciplinary team. 15. Provides preventive and rehabilitative nursing procedures to promote optimal patient health. 16. Supervises and instructs home health aides and LPNs, ensuring patient care goals are met. 17. Completes supervisory visits to the patient’s residence at least every two weeks, assessing the home health aide’s performance and patient progress. Evaluation of Outcomes of Care 18. Documents patient and caregiver responses to interventions and teaching. 19. Evaluates patient progress toward goals and modifies the plan of care as needed. 20. Ensures the plan of care is reviewed and updated at least every 60 days or when significant changes occur. 21. Notifies the physician and team of any significant changes in the patient’s condition. 22. Participates in interdisciplinary team meetings and contributes accurate, concise reports on patient progress. 23. Completes discipline discharge documentation on the day of the discharge visit, submitting all required documentation within 48 hours. Resource Utilization & Compliance 24. Ensures compliance with Medicare, Medicaid, and agency regulations. 25. Manages and orders medical supplies based on patient needs and payer requirements. 26. Documents and updates medication profiles per physician orders. 27. Ensures all verbal physician orders are documented and submitted timely. Leadership & Professionalism 28. Adheres to the Code of Ethical Behavior and maintains professional conduct at all times. 29. Participates in the agency’s Quality Assurance and Performance Improvement (QAPI) program. Outpatient Registered Nurse (RN) Care Manager, Full Time

The Outpatient Care Manager, Registered Nurse, is responsible for providing care management services to outpatients and their families as directed by the policies and procedures of the entity and Outpatient Care Management Department. The OP CM RN provides professionally established methods of assessing a patient's status of chronic and acute illnesses and assists patients and families in resolving problem areas and connects them with other members of the care team with a goal of assisting patients with self-management. Responsible for the medical complexity of patient care as it relates to medical stability and wellness, the OP CM RN collaborates with both health care and community partners to address and promote self-management of care needs. The OP CM RN also collaborates with the Outpatient Care Manager SW, and other members of the Care Management team as needed to address the social needs of the medically complex patient. JOB DUTIES AND RESPONSIBILITIES: Provides assessment, care planning and intervention to patients and caregivers, including care planning, advocacy, as well as clinical intervention as appropriate. Follows the care management process including patient identification, engagement/enrollment, assessment, care planning, and case closure. Manages a caseload of patients and prioritizes new referrals with patients who require follow up to complete care plan goals. Appropriately refers to other care manager disciplines within the department to meet the patient's holistic health care needs. Appropriately delegates tasks to the Care Manager Outreach Coordinator (CMOC) as needed. Practices motivational interviewing skills and teach-back skills when interviewing/assessment of patients. Ensures accurate clinical and patient care documentation in patient charts, completes reports and other requested/required patient documentation as needed, and maintains required statistical documentation for the department's management information system. Demonstrates competency in the assessment, range of treatment, knowledge of growth and development, and communication appropriate to the age of the patient treated. Participates in quality and/or performance improvement projects/pilots. Participates in orientation of new Care Management staff as assigned. Facilitates follow-up primary care visits within 48 hours of ED visit, urgent care appointment, or hospital discharge. Responsible for working with the patient and patient care team to develop an individualized treatment care plan – including follow-up appointments, labs and other care coordination. Tracks follow-up visits with appropriate specialists for complex patients. Communicates with and coaches’ patients to ensure that they are aware of discharge instructions; have necessary prescriptions; have access to medications and understand how to take the necessary medications, including what to look for regarding adverse events as per their care givers’ instructions. Facilitates the information flow between hospitals, long-term care facilities, home health representatives, and the patient’ s primary care team. Works with providers, clinical staff members, and clerical staff members to help identify high risk, high need patients. Assists physicians and other care team members in implementing processes for best practices in preventive services, chronic care and disease management. Utilizes electronic health record, chronic disease registry, and other quality reporting software to capably manage the care of individual patients and populations. Works collaboratively with providers and the care team to ensure patient adherence to medical plan of care, including all appropriate preventive and disease-specific screenings, interventions, and treatment goals – including self-management goals. Identifies, utilizes, and properly directs patients to cultural and community resources. Verifies that practices have necessary behavioral health screening tools. Compliant with annual network or department competencies focusing on health coaching patients on self-management tools related to chronic illnesses and appropriate health coaching. Maintain timely, accurate, complete, and consistent documentation appropriate to role in the electronic medical record. Maintains expertise in telehealth procedures, participates in staff meetings, participates in network and/or department committees or special projects as assigned. Participates in peer educational presentations. Supports Network and department goals and objectives.
