A Cath Lab RN is essential in ensuring the safety, efficiency, and effectiveness of cardiac catheterization procedures, contributing significantly to the diagnosis and treatment of cardiovascular diseases specifically with the cardiac catheterization lab. This position is on a per diem basis, meaning PD/PRN as needed. Responsibilities ** You must have experience in the Cath Lab or Critical Care area as a RN to move forward in this position. Functions as member of interdisciplinary healthcare team. Performs an initial discharge planning evaluation or Emergency Department assessment for patients. Provides supervision of Licensed Practice Nurse Navigator activities Cultivates and maintains effective interaction/communication with medical staff, nursing staff, social workers, and others to drive the care coordination process and facilitate continuity of patient care. Communicates with home health agencies, third-party payor, and other community resources as needed to coordinate needs. Facilitates clinical practice guidelines at the patient level. Performs outpatient and clinic care coordination and monitors patients' care amongst the hospital, clinic, and home. Facilitates care via telehealth to assist with patient needs and concerns. Provides health promotion activities and education for caregivers and staff at assigned facility, including workshops and trainings. Refers cases identified as risk management issues, peer review issues, or quality issues to appropriate personnel. Coordinates patient placement activities to optimize patient flow, patient outcomes, admission appropriateness, and cost efficiencies in support of organizational strategic initiatives while working with campus house supervisors, physicians, and nursing staff. Communicates and collaborates with leaders and staff at external organizations/agencies regarding placement needs for whole childcare. Maintains assigned databases. Collaborates in collection of data for identification and resolution of flow issues. Collects, analyzes, and interprets outcome data systematically to identify problem areas and assess need for improvement and/or support changes in system-wide processes in relations with the Behavioral Mental Health strategy. Prepares periodic reports as directed. Participates actively and contributes to organizational initiatives that impact patient flow. Supports continuous performance and quality improvement. Communicates openly with appropriate leadership team to ensure knowledge of key decision factors. Serves as expert clinical resource for patient placement by ensuring smooth functioning of the patient flow process to reduce wait times and avoid unnecessary patient movements. Maintains positive working relationship and demonstrates excellent customer services skills in all interactions between amongst the interdisciplinary team, physicians, internal customers, and external customers. Serves as a single point of contact for streamlining communication flow for escalated communication needs. Serves as a support liaison to the Behavioral Mental Health team.
