PURPOSE AND SCOPE: The professional registered nurse Inpatient RN CAP 2 may be an entry level designation into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or attained through advancement from CAP 1. This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. As a member of the kidney disease health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and program operations. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, applicable contractual service agreements, state and federal regulations. Performs all essential functions under the direction of the Supervisor and with the guidance from the Educator, Preceptor, or in collaboration with another Registered Nurse. Performs ongoing, systematic collection and analysis of patient data pre – during – post treatment for assigned patients and documents in the patient medical record, makes adjustments or modifications to treatment plan as indicated and notifies Supervisor, Physician, patient’s primary nurse and others as indicated. Assesses, collaborates and documents patient/family’s basic learning needs to provide initial and ongoing education to patients and family. Performs the implementation, administration, monitoring, and documentation of patient's response to prescribed intradialytic transfusions, including appropriate notification of adverse reactions to physician and appropriate blood supplier. Administers medications as prescribed and documents medical justification and effectiveness. Initiates and assists with emergency response measures. Serves as a resource, leader, coach, mentor and role model for new and incumbent employees by setting examples of appropriate behavior, work habits and attitudes towards patients, co-workers, Supervisors and the company at the facility and area level. May be asked to provide specialized nursing care instructions to hospital/facility staff as stipulated contractually. May be assigned to assist in an Outpatient facility on an as needed basis. May serve as a Preceptor to new employees. Required to complete CAP requirements to maintain or advance. Performs all other duties as assigned by Supervisor. Duties and Responsibilities: Onboarding: Assessment Communicates with new employee before start date to make introductions, identify learning style, provides an overview of the orientation process and attempt to alleviate fears and apprehensions. Diagnosis Collaborates with the Clinical Manager when a new hire is finalized and hire date is established. There will be discussion regarding new employee’s previous work history, clinical background and possible learning needs. Identification of Educational Outcomes Prepares a competency-based unit orientation for new hires based on objectives and needs of orientee. Planning Prepares a clinical orientation schedule for each new employee based on the employee’s previous work experience and clinical background. Collaborates with manager to identify a compatible preceptor and determine the anticipated length of orientation. Duties and Responsibilities: Onboarding: Assessment Communicates with new employee before start date to make introductions, identify learning style, provides an overview of the orientation process and attempt to alleviate fears and apprehensions. Diagnosis Collaborates with the Clinical Manager when a new hire is finalized and hire date is established. There will be discussion regarding new employee’s previous work history, clinical background and possible learning needs. Identification of Educational Outcomes Prepares a competency-based unit orientation for new hires based on objectives and needs of orientee. Planning Prepares a clinical orientation schedule for each new employee based on the employee’s previous work experience and clinical background. Registered Nurse (RN) – Progressive Care (Step down) – The Jewish Hospital Job Summary: The Progressive Care (Step down) Registered Nurse (RN) position is responsible for delivering exceptional nursing care to patients by: Utilizing strong organizational and leadership skills to assess patient status Acting as the service line between Med-Surg and ICU units Determining the priority of the patient’s problems and needs Developing and performing patient care plans to enhance and improve outcomes Ensuring a safe and therapeutic environment Essential Functions: Collaborate with the interdisciplinary care team Conducts initial and ongoing patient assessment, analyzes assessment data, creates a plan of care, implements treatment, and evaluates treatment effectiveness Serves as point of contact for patients' care coordination throughout hospital departments Acts as a patient safety advocate by participating in ongoing quality improvement in the department Administer medications in a safe manner consistent with the State of Practice and the organization’s policies and procedures Other duties as assigned Education: Bachelor of Science Nursing (preferred, not required) Certifications: Current state licensure as a Registered Nurse (RN) BLS Basic Life Support or ACLS Advanced Cardiac Life Support (preferred at hire, must obtain from approved source prior to direct patient care, ACLS required within 6 months of hire) Experience: 1 year of RN experience (required) 6 months RN experience in an acute care hospital setting (preferred) Mercy Health is an equal opportunity employer. Job Description Interview’s patients to determine medical problem/condition and documents in chart (EMR) for physician, when necessary obtains and records patient’s vital signs including weight and assists physician with patient examination as needed and explains procedures and treatments to patient to gain cooperation, understanding and allay apprehension, maintains awareness of comfort and safety needs, manages lipids, protimes and phone triage of patient concerns. Responsibilities Interview’s patients to determine medical problem/condition and documents in chart (EMR) for physician, when necessary obtains and records patient’s vital signs including weight and assists physician with patient examination as needed and explains procedures and treatments to patient to gain cooperation, understanding and allay apprehension, maintains awareness of comfort and safety needs, manages lipids, protimes and phone triage of patient concerns. Follows up on physician orders and lab requests, obtains direction from physician as needed and informs patient of test results and any further treatment prescribed; provides explanation and education to patient; documents services performed in EMR for billing purposes, observes patient, records significant conditions and reactions, notifies supervisor or physician of patient’s condition and reaction to drugs, treatment, and significant incidents, and assists physician in clinical care of patient. Documents nursing history in EMR and physical assessment for assigned patients and initiates a patient education plan according to the individualized needs of the patient, as prescribed by the physician. Abstracts patient records via the EMR. May sort or do chart prep on occasion but not as a routine part of the position. Uses other EMR functions such as tasking, messaging, and disease management. Uses Centricity and other software as appropriate to support nursing functions. Responds to emergency situations based upon nursing standards, policies, procedures, and protocol. Maintains crash cart and defibrillator. All other duties as assigned Qualifications KNOWLEDGE AND SKILLS: Please describe any specialized knowledge or skills, which are REQUIRED to perform the position duties. EDUCATION: Graduate of an accredited school of nursing. The Christ Hospital Health Network does hire AD and Diploma nurses; BSN preferred. YEARS OF EXPERIENCE: 3 years RN experience preferred. REQUIRED SKILLS AND KNOWLEDGE: Defibrillator, blood pressure monitor, EKG equipment, treadmill and crash cart maintenance, copier, fax, computer skills (Microsoft Outlook, Word, Excel and Centricity, EMR preferred. LICENSES & CERTIFICATIONS: Licensed RN by State Board, certified in BLS required
