Administers medications following CSHS policies and manufactures' instructions. Recognizes adverse reactions to medications and takes immediate and appropriate actions. Follows CSHS policy regarding restraint application, documentation and observation. Consistently includes Resident and family in the development of an individualized plan of care. Performs pain (5th vital sign) screening as indicated, notifies the RN immediately of patient's report of pain or discomfort, and provides appropriate non-pharmacologic interventions as directed by RN. Documents pain assessment, intervention and follow-up. Documents reassessment of pain as per hospital policy. Provides Resident/family education regarding pain. Notifies physician for unrelieved pain as indicated. Qualifications Graduate of an approved program for licensed practical nurses and licensed to practice in NJ. Previous LPN experience preferred. Valid New Jersey LPN license required. CPR certification required. Satisfactory completion of the pharmacology test is required. Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team. Overview NaphCare is hiring a Medication Assisted Treatment (MAT) LPN to join our team of medical professionals in New Brunswick, New Jersey, where we manage all aspects of healthcare for the individuals who are currently incarcerated in the Middlesex County Jail. As a Correctional Medication Assisted Treatment (MAT) LPN , you will work alongside correctional officers and other medical professionals to provide care to the individuals in residence. Working within a controlled environment, where security is top priority and the days are never dull, you will have various duties to keep you engaged including performing physical assessments, administering medications, managing chronic illnesses, and responding to emergencies. This is a unique position where you can make a difference to this diverse population of patients with complex medical needs. *New grads are encouraged to apply - we'll train you in the exciting field of correctional healthcare! * Responsibilities As an LPN , you will play a critical role in our continued mission to provide quality healthcare that protects the rights, dignity, and health of our unique patient population. Duties and responsibilities may include and not be limited to: Performing comprehensive patient assessments to determine eligibility for MAT, including medical history, substance use history, mental health evaluation, and physical examination Administering MAT medications as prescribed, ensuring proper dosing, and monitoring for adverse effects and drug interaction Educating patients about MAT medications, potential side effects, and the importance of medication adherence, addressing misconceptions and stigma Monitoring patients in the induction, stabilization, and phases of treatment patient ensuring adherence to the program Maintaining medical records, including entering information into our electronic health record, TechCare Observing patient health and mental health status Providing emergency first aid Providing patient education and discharge/release planning Qualifications Experience / Requirements / Qualifications Graduate from a Licensed Practical Nursing program High school diploma or equivalent (GED) Actively state-certified as a Licensed Vocational Nurse or Licensed Practical Nurse Must possess an active BLS CPR Certification (AHA required) Able to maintain confidentiality of all proprietary and/or confidential information Display integrity, professionalism and be able to adhere to a Code of Conduct and comply with all facility(s) correctional healthcare policies, procedures, and legal requirements Must pass the facility’s criminal background check and drug screening Physical Capabilities: Must have the ability to stand for long periods, lift and move patients (up to 50 pounds), bend, squat, reach, push, pull, walk significant distances, and perform physically demanding tasks like CPR, all while maintaining good balance and endurance throughout a shift.
